0571623625 2018-07-17 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead. Multiple contractors were on location preparing the pad for flow back operations. The production equipment consisted of nine 400-barrel tanks, two vapor recovery towers, two compressors, six sand traps, six gas production units, one vertical vent scrubber, and one combustor.
0797954301 2014-12-13 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Highlands Drilling Rig #10 drilled a 23 inch diameter hole to the depth of 478 feet and ran 18.625 inch 87.5#/ft. J-55 casing not getting past 230 feet. Pulled casing out laid down and went in the wellbore with a 23 inch tricone bit making two trips to bottom and reaming at 230 feet. Picked up casing and began running and once again hit at 230 feet. After several unsuccessful attempts to get casing past 230 feet a decision was made to pull casing, lie down and go in and ream again. Carrizo will notify ODNR when casing is picked back up and on bottom so that cement can be witnessed.

1427796851 2014-12-14 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Highlands Drilling Rig #10 drilled a 23 inch diameter hole to the depth of 478 feet and ran 18.625 inch 87.5#/ft. J-55 casing and made 2 unsuccessful attempts getting casing on bottom followed by reamer runs with a tricone bit. On the third attempt casing was landed at 444 feet. Baker Hughes was onsite and broke circulation and pumped 35bbls of gel and 10bbls of fresh water. As Baker was preparing to batch cement and found their vacuum brake was missing and one had to be brought down from Massillon. Upon arrival of Baker Hughes cemented casing with 665 sacks of Class A lead cement with 0.05% Static Free, 1% CaCl, 0.5% EC-1, 0.125#/sk. Cello Flake, 0.75gals/sk. FP-6L, 0.5% BA-10A, weighing 15.6ppg with a yield of 1.20cuft/sk. Lead cement was followed 209 sacks of Class A tail cement with 0.05% Static Free, 3% CaCl, 0.5% EC-1, 0.125#/sk. Cello Flake, 0.75gals/sk. FP-6L, and 0.5% BA-10A, weighing 15.6ppg with a yield of 1.21cuft/sk. Cement weight was verified by mud scales. Cement was displaced with 124bbls of fresh water. 60bbls of good cement returns to surface, plug was landed at 300psi and pressured to 1050psi for 5 minutes and the floats held.

Production valves were in the open position. Gauges present read 0 pounds per square inch on the 13.375-inch diameter intermediate casing and 800 pounds per square inch on the 9.625-inch diameter intermediate casing. No gauges were present on the 18.625-inch diameter surface casing. The cellar was full of water and no bubbling was present. Production equipment consisted of nine 400-barrel tanks, two vapor recovery towers, two compressors, six sand traps, six gas production units, one vertical vent scrubber, and one combustor. Identification was located at the well, at the tank battery, and at the entry.
