0106122645 2017-06-27 Completion Testing No Violations On the day of my inspection I witnessed Deep Well Services had completed drill out and were rigging down. 2.375 inch diameter L-
0372096499 2015-01-21 Drill / Deepen / Reopen No Violations On the day of my inspection, I received a call on 1/18/15 from Ken Durst Eclipse EHS Field Rep stating that the rig has experienced several kicks and were flaring. On 1/20/15 I received a call from Eclipse Representative Bruce Carpenter stating that the Precision Drilling Rig #110 had another kick at 6600 feet and spilled 3bbls of SOBM on containment around the shakers. Upon arrival I witnessed that the rig was flaring and through inspection clean-up was nearly completed around the shakers. Site inspection revealed some chemicals were off containment on the well pad. I told Jebb Dobbs Company Man to place the chemicals inside proper containment. Precision Drilling Rig #110 was currently drilling in their lateral at 10800 feet on 11.5# mud.
0396914161 2017-10-18 Production Wells No Violations On the day of my inspection I witnessed the well was equipped with a plunger lift system with valves in the open position. The production equipment included a line heater, a sand trap and a GP unit. Also present a 400 barrel purge tank within containment and 3-500 barrel frac tanks within containment and these are shared with the 4059244350000 Scott 1H. Fluids and gas are separated at the GP unit and then transferred to the Borton CGF. Gauges present read 2760 pounds per square inch on the tubing and 2940 pounds per square inch on the casing. Annular pressure monitoring was present along with proper identification displayed.
0733805350 2018-07-10 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present. The production equipment consisted of one 400 barrel tank, two indirect line heaters, two sand traps, and two gas production units. Identification was located on the well. Eclipse well tender present.

1347612818 2015-01-27 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Precision Drilling Rig #110 had reached td of 16737 feet on 12.5# SOBM for control of wellbore and ran 16708 feet of 5.5 inch 20#/ft. P-110 casing with 4 flow guns set at 16566 feet, 16511 feet, 16455 feet and 16400 feet. Schlumberger was on site and cemented casing with 1040 sacks of Class A Lead Cement with a weight of 14.2ppg and a yield of 1.47 cuft/sk. preceded by 48bbls of 13ppg Mudpush express followed by 2090 sacks of Class A Tail Cement weight of 15.6ppg and a yield of 1.26 cuft/sk. Cement was displaced with 370bbls of fresh water plug was landed at 460psi and pressured to 1028psi for 5 minutes and the floats held.
1458679622 2016-03-22 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed a slip developing on the southeast cut slope of the well pad. I placed call to Eclipse Resources and they were aware of the slip and have plans to address in the coming weeks.
1464246896 2015-01-23 Drill / Deepen / Reopen No Violations On the day of my inspection I received a call that the Precision Drilling Rig #110 took another kick at 13008 feet and they were circulating through the choke at 350GPM with no release. Upon arrival I met with Sr. Drilling Superintendent Andy Stone, Drilling Superintendent Tommy Richardson, Company Man Jebb Dobbs, Drilling Manager Dan Leonard, EHS Supervisor Bruce Carpenter, EHS Representatives Rocky King and Mike Hartshorn. We completed an extensive site inspection following the kick and release on containment which the rig took on 1/20/15. Inspection revealed housekeeping issues have been addressed and area around shakers has been thoroughly cleaned. The sump which involves NOV-776848376 is being continually pumped at this time due to damage to the sump which will not allow sump to fill above halfway. The current plan is to finish the 5H and rig down and make repairs to sump.

1496244316 2017-05-31 Completion Testing No Violations On the day of my inspection I witnessed Halliburton Services frac stage 32 of 54 with a breakdown pressure of 6818 pounds per square inch, an average treating pressure of 8749 pounds per square inch, an average rate of 88.5 barrels per minute and a total of 315379 pounds of proppant pumped. I completed walk around with Consultant Rueben Garza revealing proper secondary containment was in place and the fuel skid providing fuel for the frac job was in operation and supplied a total of 131847.6 cubic feet of natural gas for the stage.
1501693898 2017-08-02 Completion Testing No Violations On the day of my inspection I witnessed the well was flowing back with through Energes flowback equipment to sales with 2689 pounds per square inches on the tubing and 3686 pounds per square inches on the casing at 1.7 MMCF per day and 21 barrels per hour of oil and 43 barrels per hour of water through a 20/64 inch choke. I met with Consultant Cristo Giannareas and completed walk around.
1675071106 2016-07-01 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed the last activity the well was drilled to total depth on 1/24/15 and 5.5 inch production casing set at 16707 feet. Further inspection revealed the well has not been stimulated. A slip is present on the southeast cut slope of the well pad.

1911194390 2015-01-13 Administrative Inspection No Violations On the day of my inspection I received a phone call Precision Drilling Rig #110 Company Man Ryan Bartholomew stating they lost a cone off of a 12.25 inch diameter tricone bit and unsuccessfully fished for the lost cone. They were planning on setting a 300 foot kick plug from 6166 feet to 5866 feet and then kicking around plug to a planned td of 6818 feet with 9.625 inch casing to follow.
1940052905 2015-01-05 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Precision Drilling Rig #110 drilled a 17.5 inch diameter hole to the depth of 1265 feet and Wyoming Casing crew ran 1246 feet of 13.375 inch 61#/ft. J-55 Surface Casing. Schlumberger was on site and gained circulation with 200bbls of fresh water, pumped 20bbls of gel and cemented casing with 998 sacks of Class A cement 15.6ppg, 1.21cuft/sk. cement, verified by mud scales. Displaced cement with 182bbls of fresh water had good cement returns of 49bbls, plug was landed at 545psi and pressured to 1070psi for 5 minutes and the floats held. Site inspection revealed no major change in existing slip and Quantab readings from the sump were 3800ppm and 3500ppm from the ditch as clean-up is ongoing.
2086151582 2015-04-01 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by Southeast Regional Supervisor Brent Bear, I witnessed that repairs to the slip on the southwest corner of the pad were completed and a 500bbl frac tank has been placed inside containment to begin to pull fluids out of the northeast sump while a sediment pond is installed.
