0070724016 2015-02-10 Drill / Deepen / Reopen No Violations On the day of my inspection following up on NOV-776848376, I witnessed that sump repairs are completed and sampling of pad has been done revealing no contamination. NOV-776848376 is resolved and the sump will be continually monitored for sheen and pumped as needed. I met with EHS representative Rocky King and Construction Foreman Doug Rennicker to discuss repairs to the slip on southeast corner which will begin tomorrow.
0257612419 2017-08-02 Completion Testing No Violations On the day of my inspection I witnessed the well was flowing back with through Energes flowback equipment to sales with 2412 pounds per square inches on the tubing and 3600 pounds per square inches on the casing at 1.6 MMCF per day and 22 barrels per hour of oil and 43 barrels per hour of water through a 20/64 inch choke. I met with Consultant Cristo Giannareas and completed walk around.
0401900333 2015-01-13 Administrative Inspection No Violations I received a phone call from Eclipse Construction Supervisor Doug Rennicker requesting an extension on NOV-776848376 due to the current weather conditions. Doug stated that clean up should be completed on time but with the weather he would like a 2 week extension. Extension was granted until 1/26/15.
0776848376 2014-12-23 Drill / Deepen / Reopen Violations Noted On the day of my inspection I received a phone call from Eclipse Representative Gretchen Addington about a spill on the Scott Pad. Upon EHS inspection of the pad today one of the manual operated sump control valves had malfunctioned releasing a barrel of water which had oil sheen on it. Upon arrival I witnessed that the valve had been shut and isolated. Inspection revealed that sheen was present and Quantab readings inside the LOD were at 6000ppm. Instructed operator to remove contamination and remediate affected area.

1387021351 2018-07-10 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present. The production equipment consisted of one 400 barrel tank, two indirect line heaters, two sand traps, and two gas production units. Identification was located on the well.
1419437246 2014-12-24 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that a lined ditch was installed at the LOD with a pump installed pumping water to a frac tank on location. Plans were to continue until chloride levels are acceptable. Quantab readings were taken in the ditch and in the sump located on the pad and the levels were 1800ppm and 4000ppm respectively. 7500 gallons of water was removed from the sump and will be disposed of properly. Site inspection revealed two areas of laying oil inside secondary containment around the rigs air compressors. Operator was instructed to continue clean-up of area affected by the faulty valve until complete and remove oil from secondary containment.
1459073573 2016-07-01 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed the last activity the well was drilled to total depth on 12/30/14 and 5.5 inch production casing set at 17121 feet. Further inspection revealed the well has not been stimulated. A slip is present on the southeast cut slope of the well pad.
1508330416 2017-10-18 Production Wells No Violations On the day of my inspection I witnessed the well was equipped with a plunger lift system with valves in the open position. The production equipment included a line heater, a sand trap and a GP unit. Also present a 400 barrel purge tank within containment and 3-500 barrel frac tanks within containment and these are shared with the 4059244370000 Scott 5H. Fluids and gas are separated at the GP unit and then transferred to the Borton CGF. Gauges present read 2385 pounds per square inch on the tubing and 2585 pounds per square inch on the casing. Annular pressure monitoring was present along with proper identification displayed.
