Reviewed Myers Well Service run sheets with AEU Consultant Larry Henry and a total of 830bbls has been gathered at this point and taken to the rig with the current Myers Well Service truck at 20% capacity. Testing with Quantabs revealed levels at the springhouse 40.035523 -81.255717 were at 421ppm and the north roadside sump 40.035494 -81.255616 were at 665ppm with a pH of 7 and no visible solids or sheen. I then met with AEU EHS Field Representative Tom Bauer, he is having Sunpro power wash and pump out the springhouse to remove any residual solids in the bottom with further testing after cleaning.

A total of 1465bbls has been gathered at this point and taken to the rig with the current Myers Well Service truck at 60% capacity. Testing with Quantabs revealed chloride levels at the springhouse 40.035523 -81.255717 were at 203ppm and the north roadside sump 40.035494 -81.255616 were at 350ppm with a pH of 7 and no visible solids or sheen.
0916031030 2019-03-06 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with production valves in the open position. Annular pressure monitoring was present with gauges present reading 600 pounds per square inch on the 13.375-inch diameter surface casing and 1100 on the 9.625-inch diameter intermediate casing. The production equipment consisted of four sand traps, four gas production units, one vertical vent scrubber, one vapor recovery tower, and four 400-barrel tanks with vent lines plumbed into one combustor.
1016541854 2015-02-13 Drill / Deepen / Reopen Violations Noted On the day of my inspection I witnessed that Nomac Drilling Rig #81 that the rig was still tripping out of the hole after setting a 40bbl gel pill I left location. Upon return I witnessed that Nomac Drilling Rig #81 had drilled a 17.5 inch diameter hole to the depth of 1275 feet and Franks Casing crew was completing a run of 1248 feet of 13.375 inch 54.5#/ft. J---81.255527 were at 2500ppm and near Salt Fork Creek 40.034531 -81.255584 were at 3700ppm. Fluid level remained at 108 feet in the wellbore while flow remained the same at the springhouse. 1171729877 2015-03-04 Drill / Deepen / Reopen No Violations On the day of my inspection following up on NOV-1016541854, I witnessed that Nomac Drilling Rig #81 continuing to rig down. A total of 2845bbls has been gathered and taken to the rig with 1300bbls of the total coming from the rainfall since yesterday. Testing with Quantabs revealed chloride levels at the springhouse 40.035523 -81.255717 were at 188ppm and the north roadside sump 40.035494 -81.255616 were at 236ppm with a pH of 6 and no visible solids or sheen. With the current readings and rain fall pumping from the sumps has ceased. Full analytical testing results will be furnished to ODNR upon completion.
1191851762 2015-02-20 Drill / Deepen / Reopen No Violations On the day of my inspection following up on NOV-1016541854, witnessed that Nomac Drilling Rig #81was broke down waiting on another booster at depth of 6400 feet and a planned td of 7100 feet. A total of 880bbls has been gathered at this point and taken to the rig with the current Myers Well Service truck broke down at 60% capacity and a mechanic arriving getting the truck back in operation. Testing with Quantabs revealed chloride levels at the springhouse 40.035523 -81.255717 were at 400ppm and the north roadside sump 40.035494 -81.255616 were at 584ppm with a pH of 7 and no visible solids or sheen. I met with Dan Sansone of Hull & Associates as he took samples and completed field test on the springhouse with chloride levels at 365ppm and an EC of 1.98mS with the other samples getting complete testing for chlorides, metals and solids.

Production equipment included a sand trap and a GP Unit. Also present a line heater, a vapor recovery system, 1- NGL tank, a compressor unit, 2-horizontal heated separator (1 disconnected), a stabilizer unit, a vertical vent scrubber, a combustor unit and a total of 12-400 barrel tanks all within containment and shared with the 6H, 4H and 2H. Gauges present read 400 pounds per square inch on the tubing and 500 pounds per square inch on the casing with annular pressure monitoring in place.
1417714874 2014-12-04 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Precision Rathole had drilled a 36 inch diameter hole to the depth of 100 feet and ran 100 feet of 30 inch 118.65#/ft. conductor pipe. Universal Well Service was on site gained circulation and cemented pipe with 270 sacks Type 1 cement with 3% CaCl and .25#/sk. of floseal and a yield of1.2 cuft/sk. Cement was displaced with 74bbls of fresh water. No cement returns to surface instructed operator to run one inch tubing and bring cement to surface.
1417824299 2014-12-05 Administrative Inspection No Violations At 4:50 PM I received a call from AEU Drilling Superintendent Richard Thomas stating that they had ran 1 inch tubing down the backside of the conductor for remedial cement job ran some LCM, 5bbls of cement and CaCl pulled out and waited 2 hours. Land owner showed up with a sample from her spring which appeared to have gel in it. AEU shut down operations and contacted ODNR. I then contacted land owner Darlene Crooks and she stated that when she turned the water on this morning it was grey and then she went to work and upon returning the water was still discolored. She went to her spring house and it was full of grey and murky water. Complaint was taken and forwarded to ODNR Hydrologist Kyle Tharp for investigation.
1417886358 2014-12-06 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Precision Rathole had ran some LCM and Universal Well Service had pumped 2.5bbls of Type1 Cement weighing 15.6ppg with a yield of 1.19cuft/sk. through 1 tubing and waited 1 hour. Universal pumped 5bbls of Type 1 Cement weighing 15.6ppg with a yield of 1.19cuft/sk. through 1 inch tubing and waited 30 minutes. Universal pumped remaining cement for a total of 600sks with no cement to surface. Precision pumped an additional 7yds of ready mix with no cement to surface. AEU will shut down make a plan and notify ODNR before they proceed.
1423931781 2015-02-14 Casing String Witnessed No Violations I arrived on location at 10:30 AM at the request of inspector Doug Bennett. During an effort to load the hole and gain circulation, a developed spring to the south of the location was communicated with. An attempt to cement the 1248 of casing to surface was made with 1170 sacks of class H cement with .5 pounds per gallon Celloflake and 2% CaCl. Circulation could not be gained and no returns were witnessed at surface. After four hours, one inch tubing was ran inside the annular space to find the cement top. The cement top was found at 110. Once the cement top was tagged, a large amount of cotton seed hulls were poured into the annular space of the casing string. In addition, enough pea gravel was placed to cover twelve feet of the annular space. While placing the lost circulation materials the annular space was bridged off at twenty feet from surface. Several attempts to clear the obstruction were made with the one inch tubing. O-Tex then placed twenty two barrels of class H cement on top of the obstruction causing it to move downhole approximately fifteen feet. The spring that was impacted, was flowing much clearer at the time of my departure than when I arrived and the chlorides were down to 2400 ppm.

Reviewed Myers Well Service run sheets with AEU Consultant Larry Henry and a total of 760bbls has been gathered at this point and taken to the rig with the current Myers Well Service truck at 35% capacity. Testing with Quantabs revealed levels at the springhouse 40.035523 -81.255717 were at 533ppm and the north roadside sump 40.035494 -81.255616 were at 820ppm with a pH of 6 and no visible solids or sheen.

A total of 1125bbls has been gathered at this point and taken to the rig with the current Myers Well Service truck at 40% capacity. AEU EHS Field Tech Amy Louwers stated full analytical testing results will be furnished to ODNR next week with all initial test levels within normal ranges. NOV-1016541854 is resolved.

The other sumps were frozen over with no flow reaching the south side of the Oxford Road. A total of 710bbls has been gathered at this point and taken to the rig with the current Myers Well Service truck at 40% capacity.
1906117493 2016-06-23 Production Wells No Violations On the day of my inspection I met with Ascent Resources Production Foreman Kelly Swope and witnessed Cameron Services rig up their choke manifold and relief the pressure on the between the 9.625 inch casing and the 5.5 inch casing. Pressure was relieved from 1900psi to 620psi and shut-in for continued monitoring.
