During my inspection the well was equipped with a production tree style wellhead with production valves in the open position. Annular pressure monitoring was present with gauges present reading 630 pounds per square inch on the 13.375-inch diameter surface casing and 250 on the 9.625-inch diameter intermediate casing. The production equipment consisted of four sand traps, four gas production units, one vertical vent scrubber, one vapor recovery tower, and four 400-barrel tanks with vent lines plumbed into one combustor.

Also present a line heater, a vapor recovery system, 1- NGL tank, a compressor unit, 2-horizontal heated separator (1 disconnected), a stabilizer unit, a vertical vent scrubber, a combustor unit and a total of 12-400 barrel tanks all within containment and shared with the 8H, 6H and 2H. Gauges present read 275 pounds per square inch on the tubing and 480 pounds per square inch on the casing with annular pressure monitoring in place.
1235533303 2014-12-11 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that the TOC was at 48 feet and Precision Rathole set a pea gravel bridge from 48 feet to 43 feet topped with LCM. Express Energy Services was on site and pumped cement in increments successfully bringing cement to surface.

-55 Surface Casing washing down the last 20 feet. Nomac Drilling Rig#81 gained circulation and O-Tex Pumping LLC was on site and pumped 50bbls of gel, 10bbls of fresh water and began cemented casing at 102bbls into the cement job the pump truck broke down. Nomac Drilling Rig #81 hooked their rig pumps up and circulated cement out wellbore. Nomac Drilling Rig #81 will continue to circulate until a new pump truck and more cement can be delivered.

1923200799 2014-12-10 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Precision Rathole had drilled a 36 inch diameter hole to the depth of 100 feet and ran 100 feet of 30 inch 118.65#/ft. conductor pipe. Universal Well Service was on site doing a balanced job and cemented pipe with 150 sacks Type 1 cement with 3% CaCl and .25#/sk. of floseal and a yield of 1.2cuft/sk. Cement was displaced with 74bbls of fresh water with no returns and top of cement was at 40 feet. Second part of balanced job will be completed tomorrow.

2146698318 2014-12-15 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Southeast Regional Manager Brent Bear, I witnessed that the mouse hole had been completed with cement to surface and primary rig containment was being installed. I made phone call to AEU Construction Supervisor Randy Long and discussed erosion issues along access road he stated that the hydro seeding that took place was to keep things stable until the ground dried out to allow access for repairs.
