0224734455 2014-09-30 Casing String Witnessed No Violations Highland rig 10 is running 520of j-55, 87.50lb/ft of 18.625casing. They ran a double float shoe with a stinger on drill pipe. There is a18.625centrilizer at every joint. They ran drill pipe of 4.5,16.60lb/ft. to the float shoe seat and start cement job. Baker Hughes is the cementing contractor. The open hole is 23 and at a depth of 550. They pumped 605 sacks of regular class A cement @3%CaCl with 1/4lb per sack of celloflake. Plug down at 20:30 hrs.
0352516357 2018-02-28 Drill / Deepen / Reopen No Violations I received a call from ODNR Engineering Tech Wayne Cottle concerning contamination found on the Mohorovich Pad. Upon arrival I witnessed Ascent is re-constructing the pad to meet their specs and while digging up the perimeter drain oil based drilling mud (OBDM) was found. An area of 3 feet by 3 feet by 385 feet was found to be saturated with the OBDM. I met with Ascent Construction Supervisor Randy and CESO Construction Project Superintendent Gary Davis where the contamination was discussed. All contamination will be put in roll off boxes, tested and disposed of in an approved landfill. Construction will continue in preparation for their drilling rig to arrive. New berms and perimeter drains will be installed as new 10 foot cellars are now in place.

The cellar has been filled with pea gravel to the surface with a 6 inch polyvinyl chloride snorkel for water extraction.
1167214663 2019-04-30 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead. Production tubing valves were in the open position. Pressure monitoring was present the 13.375-inch diameter intermediate casing had 0 pounds per square inch, 9.625-inch diameter intermediate had 100 pounds per square inch, 5.5-inch diameter production casing had 2070 pounds per square inch, and the production tubing had 1250 pounds per square inch. The production equipment consisted of ten 400-barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, and one combustor. The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.
1409784223 2014-09-03 Casing String Witnessed No Violations On the date of inspection I was informed my the company man on duty that a 30 inch diameter hole was drilled by Lightning Energy Services to a depth of 67 feet. Lightning then set 24 inch casing with a weight of 94.62lbs/ft on bottom. Allied Oil and Gas Services pumped 135sk of Class A cement weighing 15.6ppg, with a yield of 1.18, 2% CaCl, and 0.25lbs/sk of flake. They displaced with 29bbl of fresh water. 5bbl of cement returned to surface. While on site the cement sunk approximately 36 inches below the bottom of cellar and was topped off with cement. The cement sank to approximately 30 inches below the bottom of the cellar while I was on site.
1424799308 2018-11-09 Completion Testing No Violations Status check inspection found the well drilling and completion process is now complete. The well was stimulated in 73 stages of 151 total stage length average. The 5.5 diameter casing has a bottom toe sleeve that was set at 19,297. There is a 7.0625 primary master valve on the well. A service rig is on location and rigged up on the # 14 well drilling out the completion plugs and running the production tubing. Crews are on site installing the production equipment, 2 tank batteries are being constructed on site.
1470331088 2019-02-04 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead and a plunger lift system. The plunger lift system was plumbed up, but the wells were flowing without the assistance of the plunger. Production tubing valves were in the open position. Gauges were present on all casing strings and they read as follows, 13.375-inch diameter surface casing had 0 pounds per square inch, 9.625-inch diameter intermediate had 250 pounds per square inch, 5.5-inch diameter production casing had 2850 pounds per square inch, and the production tubing had 2700 pounds per square inch. The production equipment consisted of ten 400-barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, and one combustor. The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry. Wise tanker truck, USA Compressions, and Ascent Company well tender were all on location. I called Jarod Cogar with Ascents production to make him aware of the pressures found on the 9.625-inch diameter by 5.5-inch diameter annulus on the 1H well and stated that our records indicate that 18.625-inch diameter surface casing was ran on these wells and there were not any gauges present at surface to monitor pressures. I asked that Ascent look into getting a gauge and pressure relief valve assembled on the 18.625-inch casing.
1646463108 2017-08-07 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed there was no change in the status of the well since the last inspection on September 19, 2016 with the 9.625 inch diameter intermediate secured with a bolted night cap. There is no production equipment, flowline or salesline in place and the location is shared with the Mohorovich 1S, 2S, 3S, 8H and Rector 14H.

During my inspection Patterson 577 was drilling the 8.5-inch diameter wellbore within the lateral section at a measured depth of 19,328 feet. The rig was utilizing a 11.8 pound per gallon drilling mud. The proposed measured depth of the well was 19476 feet.
1887514915 2015-01-14 Drill / Deepen / Reopen No Violations On the day of my inspection following up from release on Sunday, clean up was completed satisfactory. Patterson UTI Drilling Rig #334 was skidding to the said well with plans to pick up tools later this evening.
1924445934 2015-01-22 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson Drilling Rig #334 had set the 2nd Intermediate casing at 6847 feet and during displacement did not bump plug. After pressuring up to 650 and leaving shut in 6 hours Patterson Drilling Rig #334 tripped in the hole and tagged cement at 6241 feet inside casing. The rig is currently in the process of walking back to the 1H to begin to laydown drill pipe as the rig will not complete the laterals at this time it will begin to stack out.
