1411321507 2014-09-21 Casing String Witnessed No Violations On the day of my inspection I witnessed that Alpha Hunter Drilling Rig #9 had drilled a 22inch diameter hole to the depth of 439 feet and ran 439 feet of 18.625 inch 87.5#/ft. J-55 casing. Halliburton was on site gained circulation with 165bbls fresh water then pumped 25bbls of gel, 10bbls of fresh water and cemented casing with 714 sacks of Class A lead cement with a weight of 15.6ppg verified by mud scales and a yield of 1.18 followed by 200 sacks of BondCem cement with a weight of 15.8ppg verified by mud scales and a yield of 2.31cuft/sk. Cement was displaced with 115bbls of fresh water. Good cement returns of 147bbls to surface.
1460725958 2019-04-30 Completion Testing No Violations The purpose of my inspection was a status check on completion operations. During my inspection the well had just recently had 113 stages of frac plugs drilled out and production tubing installed. A service rig was on location rigged up on the 7H well drilling out frac plugs.

1557767167 2019-05-13 Completion Testing No Violations The purpose of my inspection was a status check on fishing operations. 2.875-inch diameter tubing was being utilized to drill out frac plugs and after drilling out the last frac plug the bottom of the tubing string was at 24,500-feet and the tubing string parted at 675-feet. Deep Well standalone snubbing unit 15 was rigged up on the well. I spoke with Eclipse Company Man Don Barker and he stated that the snubbing unit had just successfully latched on to the tubing string.

1864518136 2018-11-08 Production Wells No Violations The purpose of my inspection was to witness temporary abandonment activities on well #2-4415. At the time of my inspection I met with Eclipse Resources Environmental, Health, Safety and Regulatory Manager Bruce Carpenter to discuss the operations for the day and review the submitted and approved SIMOPS plan. The surface equipment on location would be outside of the fall radius of the drilling rig. Well #2-4415 had tubing isolation plugs set at 1740 and 1706 inside the tubing due to failed attempts to set plugs in the profile nipples. I witnessed the residual tubing pressure bled off prior to the setting of the back pressure valve and one of two valve recovery plugs in the well head. The intermediate and production casing annular pressure gauges were plumbed above grade and labeled. Both gauges indicated 300 psi. The gauges would remain above grade during operations. Following the temporary abandonment of the well, a steel cage would be placed over the well head.
