0153649791 2015-04-29 Preliminary Restoration No Violations On the day of my inspection accompanied by Southeast Regional Supervisor Brent Bear, the site passed preliminary restoration according to ORC 1509.072A.
0228152879 2017-07-10 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed H&P Flex Rig #608 was drilling ahead on 11.9 pound per gallon mud in their curve at 7465 feet which will be landed at 7819 and then completing the lateral section to 18900 feet. I met with Eclipse Consultant Matt Hayes and completed walk around.
0252676719 2019-01-22 Drill / Deepen / Reopen No Violations The inspection revealed that there was a cage over the existing well and it was raised up to install a gauge at the top of the production flange. The pressure measured 0 psi. The intermittent annular pressure was at 480 psi and the surface annular pressure was at 300 psi. I was accompanied by Austin Gutridge of Oil and Gas Division of Department of Natural Resources and Brad Paisley of Eclipse. The gas is being feed from the pipe line and there are two down hole barriers in the bottom of the tubing and one in the tubing hanger close to surface.
0296317913 2019-01-17 Production Wells No Violations The inspection revealed that the well is shut in under simultaneous operations. The well equipped with no way to read the pressure at the production tubing. The surface casing pressure measured 260 psi and the intermediate casing measured 400 psi. There was a steel cage over the well. I called company and Brad Paisley would be out on 1-22-19.

The well is being monitored at the intermediate string with 450 psi on the gauge and 300 psi on the gauge on the surface string. The well is isolated and the gas feeding the rig is coming from the flowline. Nathen Anderson of Eclipse accompanied me while looking at the well pressures.
0542698964 2019-05-13 Production Wells No Violations During my inspection the well was equipped with a work over valve. The temporary plugs have been removed and the well is awaiting to be flowed back to sales. Deep Well Snubbing Unit 15 was rigged up on the 3H well.
0550322277 2017-10-02 Completion Testing No Violations On the day of my inspection I witnessed Deep Well Service Stand Alone Snubbing Unit # 6 (DWS # 6) was drilling out frac plug #29 of 69 at 12101 feet with a 4.5 inch diameter tricone bit. Mid-Atlantic Services was onsite controlling well pressure at 1300 pounds square inch along with Energes Services assisting with flowback of fluids.
0579116169 2017-07-07 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed H&P Flex Rig #608 was completing rig up with plans to re-spud well this evening. H&P Flex Rig #608 will pick up Baker Hughes Autotrak trip in the wellbore and drill out the 9.625 inch diameter casing float equipment to the kick off point of 6879 feet, build their curve and drill their lateral to a planned total depth of 18918 feet. I completed walk around with Eclipse Consultant Ryan Bartholomew.
0794338404 2014-08-25 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Alpha Hunter Drilling Rig #9 had drilled a 24inch diameter hole to the depth of 438 feet and ran 438 feet of 18.625 inch 87.5#/ft. J-55 casing. Halliburton was on site gained circulation with 210bbls fresh water then pumped 25bbls of gel, 10bbls of fresh water and cemented casing with 425 sacks of BondCem cement with a weight of 15.8ppg verified by mud scales and a yield of 2.31cuft/sk. Cement was displaced with 120bbls of fresh water. Had good cement returns of 28bbls and cement stayed at surface.
0809354354 2019-05-28 Production Wells No Violations The purpose of my inspection was a status check. The well had been previously temporary abandoned due to simultaneous operation permit conditions. During my inspection the well was being prepared to be put back online. The well was uncaged, and the plugs set within the tubing and wellhead were removed. Snubbing Unit rig number 15 was being hauled off location. Identification signs were posted at entry.
1090330967 2017-09-01 Completion Testing No Violations On the day of my inspection I witnessed Halliburton Services complete frac stage 4 of 69 with a breakdown pressure of 6553 pounds per square inch, an average treating pressure of 8815 pounds per square inch, an average rate of 81.2 barrels per minute and a total of 330270 pounds of proppant pumped. Nine Wireline was onsite to plug and perforate stage 5 of 69. Halliburton pumped tools down with 506 barrels of produced water at 16 barrels per minute with 6602 pounds per square inch. Nine Wireline successfully set the plug at 18229 feet and perforated the casing with 36 shots from 18206 feet to 18082 feet. I completed walk around with Eclipse Consultant Chris McCullough revealing proper secondary containment was in place.

During my inspection the well was equipped with a plunger lift system with valves in the open position. The production equipment consisted of one 400 barrel tank, two sand traps, and two gas production units.

1461947807 2019-01-22 Production Wells No Violations During my inspection today, I was accompanied by Pat Shreve. H&P Drilling Rig 531 was on location tripping the drill pipe from 34059244170000. This well was equipped with two plugs downhole within the production tubing and Valve Removal plugs within the wellhead to shut of the pressure from the 5.5-inch diameter production casing. A production foreman for Eclipse, Brad Paisley, met us on location to evaluate the well pressures. Mr. Paisley placed a gauge on top of the well to show us that the tubing pressure remined zero. The gauge on the 9.625-inch diameter intermediate consisted of 480 pounds per square inch and the gauge on the 13.375-inch diameter surface casing consisted of 300 pounds per square inch.

1541772131 2018-11-08 Production Wells No Violations Accompanied by ODNR Ken Frye and during our inspection we met with Eclipse Bruce Carpenter to discuss temporarily abandoning the Fineran 2H well to finish drilling 4 additional wells on pad. At this point 2 pack off plugs have been set within the production tubing at 1740 feet and 1706 feet. Contractors were on location today preparing to set the valve removal plugs within the wellhead. HP 531 is scheduled to arrive on location by the end of November.

1823145384 2017-07-17 Casing String Witnessed No Violations On the day of my inspection I witnessed that after H&P Flex Rig #531 had reached a total depth of 18999 feet, casing was set at 18983 feet set at 4.4 feet above KB. KB height is 26.8 feet above ground level. Schlumberger was onsite and was pumping the last 23 barrels of lead cement and 178.7 barrels of tail cement. Lead cement was 2105 sacks of Class A 50/50 POZ weighing 14.8 pounds per gallons and a yield of 1.13 cubic foot per sack. The Tail cement was 865 sacks of Class A 50/50 POZ weighing 14.8 pounds per gallon and a yield of 1.16 cubic foot per sack. Cement weights were verified by pressurized mud scales throughout the job. Plug was dropped and cement was displaced with 419 barrels of water. Plug was landed at 2560 pounds per square inch and pressured up to 3220 pounds per square inch for five minutes and the floats held with 5 barrels back. 29 barrels of Mudpush was circulated to the surface.
1864139442 2017-10-16 Production Wells No Violations On the day of my inspection I witnessed that flowback operations were completed and the well was now online. The well was equipped with a plunger lift system with valves in the open position. The production equipment included 2-sand traps, 2-GP units and a 400 barrel purge tank. Fluids are separated at the GP unit and then transferred to the Borton CGF. Gauges present read 2235 pounds per square inch on the tubing and 2320 pounds per square inch on the casing. Annular pressure monitoring was present along with proper identification displayed. I completed walk around with well tender Scott Stemm reviewing the wells performance, while fencing was being installed and Energes Flowback Services rigging down.
