0358980296 2018-10-04 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present and evaluated. The production equipment consisted of eight 400-barrel tanks, two vapor recovery towers, five sand traps, five gas production units, one compressor, one vent scrubber, and one combustor. Identification was located on the well.

Tetra Technologies was onsite setting flowback equipment with flowback to begin on Friday. Production equipment included a sand trap, a line heater and a GP Unit. Also present 8-400 barrel tanks within containment, a horizontal heated separator, 2- vapor recovery towers, 2-compressor units, a flare stack and a gas combustor all shared with the Abe Troyer 2H, 4H, 6H and 8H. I met with Consultant Cristo Giannareas and completed walk around revealing installation of production equipment was on going.

0597484580 2019-05-20 Production Wells No Violations The purpose of my inspection was a status check. On 4/25/2019 inspection found two slips have developed along the access road and on the topsoil stockpile. I contacted ODNR horizontal well pad Engineer Mark Atkinson and made him aware of the problem areas. During my inspection today the slips remain but have not increased in severability. The current slips will not effect pad stability and performance.
0611060016 2016-10-24 Completion Testing No Violations On the day of my inspection following up on Complaint ID 5456, I witnessed the well was equipped with a Sweco tubing head and a FMH frac valve stack with all valves in the closed position. I met with Eclipse Consultant Will Ochampaugh who stated the well has not been fraced yet and completed walk around revealing proper secondary containment was in place. I then reviewed the bond log for the 5.5 inch casing provided by Nine Wireline revealing the production casing was cemented in accordance with OAC 1501:9-1-08 (7) (a) (i).

1438553194 2014-10-26 Casing String Witnessed No Violations I was onsite to witness the cementing of the surface casing. When I arrived onsite they were washing down the last 15ft of pipe. Nomac 359 has ran there 13-5/8 surface casing to a depth of 1408. Halliburton was onsite to cement the pipe back to the surface. They used a total of 1435sks of cement with 32bbls return to the surface.
