0510899681 2019-04-25 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present and evaluated. The production equipment consisted of eight 400-barrel tanks, two vapor recovery towers, five sand traps, five gas production units, one compressor, one vent scrubber, and one combustor. Identification was located on the well. Upon entering the access road, a slip was noticed developing on the left side of the access road at the point of entry and an additional slip had developed on the stack pile. I made ODNR Engineer Mark Atkinson aware of my findings.
0535431487 2016-11-10 Completion Testing No Violations On the day of my inspection I witnessed the drill out operations were completed as Deep Well Services Rig #4 was rigging down preparing to move off location. I completed walk around with Consultant Cristo Giannareas revealing that secondary containment was being repaired and replaced by Grady Rentals along with cleaning of location. Halliburton Services are scheduled to move back in to complete the frac operations of the 8H and 10H next week.

1417429307 2014-12-01 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #359 had drilled a 17.5inch diameter hole to the depth of 1340 feet and ran 1340 feet of 13.375 inch 61#/ft. J-55 casing. Halliburton was on site and washed down the last 25 feet of casing then gained circulation and pumped 15bbls of gel, 10bbls of fresh water and cemented casing with 1230 sacks of Class A cement with 2% CaCl, weighing 15.6ppg verified by mud scales and a yield of 1.21cuft/sk. Cement was displaced with 197bbls of fresh water. Good cement returns of 30bbls, plug was landed at 600psi and pressured to 1184psi for 5 minutes and the floats held.
1656292080 2017-01-10 Completion Testing No Violations The well was equipped with a plunger lift system with valves in the closed position with annular pressure monitoring in place. Tetra Technologies was onsite setting flowback equipment with flowback to begin on Friday. Production equipment included a sand trap, a line heater and a GP Unit. Also present 8-400 barrel tanks within containment, a horizontal heated separator, 2- vapor recovery towers, 2-compressor units, a flare stack and a gas combustor all shared with the Abe Troyer 4H, 6H, 8H and 10H.
