0310023517 2015-02-20 Completion Testing No Violations On the day of my inspection I witnessed that Deep Well Service completion rig was running 2.875 inch 6.5#/ft tubing at 3000 feet setting it at 7533 feet and JPS was controlling the well at 2000psi. Completed walk around with Consultant Joe Watt revealing production equipment was being installed and proper containment was present. I instructed operator to pull standing fluid out of inside secondary containment so that it would not be tracked onto the well pad.

Also present a GP Unit and a sand trap. The well was equipped with a plunger lift system with valves in the open position with 300 pounds per square inch on the tubing and 800 pounds per square inch on the casing with annular pressure monitoring in place.

Production equipment included a sand trap, a GP Unit, a line heater, a vapor recovery system, 2-NGL tanks, 2 compressor units, a combustor unit and a total of 12-400bbl tanks all within containment and shared with the 2H, 6H and 8H. The site passed preliminary restoration according to ORC 1509.072A.

1412258029 2014-10-02 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Nomac Drilling rig 80 had drilled to a total depth of 15944. They were in the process of running new 4.5 production casing. I walked the drill site and found no leaks or spills inside or outside of the containment system. The permit was posted in the company mans trailer and at the site entrance. There were no reports of hydrogen sulfide gas while drilling.
1494810002 2014-09-18 Casing String Witnessed No Violations On the day of my inspection I found that Nomac Drilling rig 80 had drilled a surface hole on air to a depth of 435, where they ran new, centralized 13.375 casing. O-Tex Pumping cemented the casing to surface with 660 sacks of class H cement. Good circulation was maintained throughout the job with approximately 27 barrels of cement returns.

1812481600 2015-08-10 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a production tree with valves in the closed position. Casing pressure was at 2150psi and tubing pressure at 2050psi. Production equipment included a sand trap, a GP Unit, a line heater, a vapor recovery system, 2-NGL tanks, 2 compressor units, a combustor unit and a total of 12-400bbl tanks all within containment and shared with the 2H, 6H and 8H SEI Oilfield Services was on site installing buildings over the compressor units and Advanced Oilfield Services was also on site replacing gaskets in the heater treater. Completed walk around with production supervisor Jeff Glenn revealing all wells shut-in with proper lock out tag out in place and containment under the heater treater.
1845663111 2018-12-18 Production Wells No Violations The purpose of my inspection was a follow up on a release of produced fluid. During my inspection there were a total of 4 production wells on pad. The well consisted of a plunger lift system with production valves in the closed position and 0 pounds per square inch present on the 13.375-inch diameter surface casing and 250 pounds per square inch on the 9.625-inch diameter intermediate casing. The production equipment consisted of four sand traps, four gas production units, one heated horizontal separator, two oil stabilizer units with reboilers, twelve 400-barrel tanks, one vapor recovery tower, and one combustor. All equipment appeared to be assembled and ready for use. A 60 feet by 60 feet area was surrounded by caution tape where the release occurred. One lined roll off container was located by the area of the release. Identification was located on the well, on the tank battery, and posted at the entry along with laminated drilling permits.
