0361063981 2018-09-10 Completion Testing No Violations The purpose of my inspection was a status check on hydraulic stimulation operations. During my inspection Frac Tech Services and Nine Energy Wireline was on location. 57 stages of 75 have been completed.
0755022521 2017-08-07 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed there was no change in the status of the well since the last inspection on October 27, 2016 with the 9.625 inch diameter intermediate secured with a bolted night cap. There is no production equipment, flowline or salesline in place and the location is shared with the Mohorovich 1S, 2S, 3S, 4S and the 8H.
0841376152 2019-04-30 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead. Production tubing valves were in the open position. Pressure monitoring was present the 13.375-inch diameter intermediate casing had 0 pounds per square inch, 9.625-inch diameter intermediate had 520 pounds per square inch, 5.5-inch diameter production casing had 2320 pounds per square inch, and the production tubing had 1800 pounds per square inch. The production equipment consisted of ten 400-barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, and one combustor. The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.

1310244575 2014-12-28 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Patterson UTI Drilling Rig #334 was drilling ahead on air at 6752 feet when making a connection lost a 12.25 inch diameter bit, a 1.75 mud motor, and 3-8 inch drill collars. Patterson UTI Drilling Rig #334 had screwed back into the fish and unsuccessfully jarred on it for 2 days and was currently tripping out of the hole and will trip back in with wash pipe with plans to wash over fish at 6580 feet tomorrow morning and try again to retrieve the fish.
1371614101 2019-02-04 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead and a plunger lift system. The plunger lift system was plumbed up, but the wells were flowing without the assistance of the plunger. Production tubing valves were in the open position. Gauges were present on all casing strings and they read as follows, 13.375-inch diameter surface casing had 2 pounds per square inch, 9.625-inch diameter intermediate had 800 pounds per square inch, 5.5-inch diameter production casing had 1302 pounds per square inch, and the production tubing had 2800 pounds per square inch. The production equipment consisted of ten 400-barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, and one combustor. The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry. Wise tanker truck, USA Compressions, and Ascent Company well tender were all on location. I called Jarod Cogar with Ascents production to make him aware of the pressures found on the 9.625-inch diameter by 5.5-inch diameter annulus on the 1H well and stated that our records indicate that 18.625-inch diameter surface casing was ran on these wells and there were not any gauges present at surface to monitor pressures. I asked that Ascent look into getting a gauge and pressure relief valve assembled on the 18.625-inch casing.
1418748712 2014-12-16 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Patterson UTI Drilling Rig #334 had ran and set their 2nd Intermediate casing at 6678 feet on the Mohorovich 2H 34059244210000. Successfully skidded and drilled out the float equipment on the Rector East 14H at 1867 feet and drilling ahead on air at 1931 feet. Completed walk around with Company Man Skip McLelland revealing no violations

1511630671 2014-09-15 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Whipstock Ohio Drilling Rig #1 drilled a 17.5 inch diameter hole to 1857 feet and had ran 1828 feet of 13.625 inch casing and cemented to surface. Whipstock Ohio Drilling Rig #1 has been released and Carrizo is waiting on Highlands Drilling Rig #10 to rig up on the Mohorovich #4H.
1541781915 2018-11-09 Completion Testing No Violations On site inspection found 6 total wells on the location that have all had the drilling and completion process complete. The said well has a stand alone completion service rig snubbing unit rigged up on it, the rig is owned by Deep Well Services. The rig is pulling out of the hole with a overshot recovery in attempt to recover a lost wireline perforating gun that was lost on stage 18 of the completion process. Nine energy wireline service is standing by to recover the gun that is inert, the charges have been detonated. The rig has 240 joints to recover. 75 total stages were completed. The 5.5 diameter production string has 3 toe sleeve pressure activated open devices set in the end of the string with the last sleeve at 19,488. All additional wells have isolation plugs set in the well bore over the last stage being completed along with a single 7.0625 diameter well control completion valve. The company has contractors on site installing the production equipment which will include (2) tank batteries. The company is indicating the wells are in the wet gas window and will produced oil.

1795669969 2016-09-07 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed no current activity and the drilling permit expires on 12/17/16. The last activity was on 1/3/15 with the 9.625 inch intermediate being set at 6720 feet. The cellar has been filled with pea gravel to the surface with a 6 inch polyvinyl chloride snorkel for water extraction.
2060988646 2014-09-07 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Whipstock Ohio Drilling Rig #1 drilled a 23 inch diameter hole to the depth of 482 feet and ran 482 feet of 18.625 inch 87.5#/ft. J-55 casing and stabbed the float shoe with 4.5 inch diameter drill pipe. Whipstock broke circulation with 89bbls of fresh water. Baker Hughes was on site and pumped 35bbls of gel, 10bbls of fresh water and cemented casing with 480 sacks of Class A lead cement with 0.05% Static Free, 1% CaCl, 0.5% EC-1, 0.125#/sk. Cello Flake, 0.75gals/sk. FP-6L, 0.5% BA-10A. Lead cement was followed 125 sacks of Class A tail cement with 0.05% Static Free, 3% CaCl, 0.5% EC-1, 0.125#/sk. Cello Flake, 0.75gals/sk. FP-6L, and 0.5% BA-10A. Displaced cement with 6.8bbls of fresh water. Had good cement returns of 43bbls to surface.
