The site passed preliminary restoration according to ORC 1509.072A.

The well is online and equipped with a production tree with valves in the open position. Gauges present read 1900 pounds per square inch on the casing and 1600 pounds per square inch on the tubing. Production equipment included a sand trap, a GP Unit, a line heater, a vapor recovery system, 2 compressor units, a combustor unit and a total of 10-400 barrel tanks all within containment and shared with the 2H and 4H. 1463158874 2016-05-13 Production Wells No Violations On the day of my inspection I witnessed the well remained shut-in with re-routing of the gas salesline in progress. Crown Oil and Gas, Anderson Excavating, Advance Oilfield Solutions and SEI Oilfield Services were onsite making repairs. Instructions were given to Crown employees to move and meet a 100 feet setback distance from the production tanks for the welding, grinding and cutting of the new line.

1811689004 2016-05-02 Production Wells No Violations On the day of my inspection I received a call from ODNR Engineer Mark Atkinson about a slip that had developed on the Lawson Pad. Arriving on location I witnessed the southwest corner of the pad had a significant slip measuring 148 feet on the pad funneling down to 90 feet wide at the base. I met with Ascent Production Supervisor Adam Pence and Construction Supervisor Randy Long and discussed the slip. Mr. Pence stated that the well was shut-in with the no pressure on the flowlines and fluids were being removed from the production tanks. The NGL tanks were empty and disconnected. The gas flowline will be disassembled and blanked off where repairs were needed. Ceso Inc. was onsite doing preliminary assessment of the area with an engineered plan to be prepared and sent to ODNR for approval of the material modification of the pad.

Anderson Excavating and Geo Technology Associates were on site performing core sampling to determine slope stability.

Annular pressure monitoring was present with 0 pounds per square inch on the 9.625-inch diameter intermediate casing and 0 pounds per square inch on the 13.375-inch diameter surface casing. The production equipment consisted of three sand traps, three gas production units, one heated horizontal separator, one oil stabilizer unit with reboiler, five 400-barrel tanks, one vapor recovery tower, and one combustor. Identification was present.
