0106372075 2017-02-07 Production Wells No Violations On the day of my inspection I witnessed the production equipment included 12-400 barrel tanks within containment, 2-NGL tanks, a vapor recovery tower, 2-compressor units, 2-horizontal heater treaters, 2-oil stabilizer units and a gas combustor all shared with the Caston 4H, Caston 6H and Caston 8H. Also present a GP Unit and a sand trap. The well was equipped with a plunger lift system with valves in the open position with 350 pounds per square inch on the tubing and 900 pounds per square inch on the casing with annular pressure monitoring in place.
0408164403 2015-04-23 Production Wells No Violations On the day of my inspection I witnessed that the well is online and equipped with a Universal Wellhead LLC casing and production wellhead along with a Universal Wellhead LLC production tree with valves in the open position. Casing pressure was at 2550psi and the tubing was a 2250psi. Production equipment included a sand trap, a GP Unit, a line heater, a vapor recovery system, 2-NGL tanks, 2 compressor units, a combustor unit and a total of 12-400bbl tanks all within containment and shared with the 4H, 6H and 8H.
0425556312 2015-01-16 Completion Testing No Violations On the date of inspection I am on site to investigate a diesel fuel spill along the access road to the Caston Pad. The Caston Pad is currently undergoing stimulation. ODNR Inspector Doug Bennett was informed of the diesel fuel spill by Tom Bauer with American Energy Utica (AEU). At the time of my inspection I found that there was a diesel fuel spill on the access road to the Caston Pad which intermittent over approximately 2 tenths of a mile. The spill was between 1 and 2 feet wide along the length of the spill and was located on the left hand side of the road if facing uphill.Tom Bauer was on site trying to determine which truck/company had leaked the diesel fuel along the road. Bauer initially believed the spill came from a Pilot fueling truck. At the time of my inspection Bauer had dispatched Pilot and W. Pol Excavating to aid in the cleanup. W. Pol was contracted to excavate the contaminated gravel along the road and replace it with new gravel. After some investigating by Bauer it was determined that a Halliburton Pump Truck had leaked the diesel fuel along the road and while I was on site I was informed of a similar spill on the Red Hill Farm Pad nearby. It was estimated that the spill was approximately 2bbl. Refer to inspection report from the same date concerning the Red Hill Farm spill.
1094721307 2015-08-10 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a production tree with valves in the closed position. Casing pressure was at 1950psi and tubing pressure at 1900psi. Production equipment included a sand trap, a GP Unit, a line heater, a vapor recovery system, 2-NGL tanks, 2 compressor units, a combustor unit and a total of 12-400bbl tanks all within containment and shared with the 4H, 6H and 8H. SEI Oilfield Services was on site installing buildings over the compressor units and Advanced Oilfield Services was also on site replacing gaskets in the heater treater. Completed walk around with production supervisor Jeff Glenn revealing all wells shut-in with proper lock out tag out in place and containment under the heater treater.
1098672408 2015-03-23 Preliminary Restoration No Violations On the day of my inspection I witnessed that the well is online and equipped with a production tree with valves in the open position. Production equipment included a sand trap, a GP Unit, a line heater, a vapor recovery system, 2-NGL tanks, 2 compressor units, a combustor unit and a total of 12-400bbl tanks all within containment and shared with the 4H, 6H and 8H. The site passed preliminary restoration according to ORC 1509.072A.

Also present a GP Unit and a sand trap. The well was equipped with a production tree with valves in the open position with 1000psi on the tubing and 1750psi on the casing.

1536488638 2014-10-17 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Nomac Drilling Rig 80 had reached a total depth of 14374. They were in the process of tripping out with the drill string. I found no leaks or spills around the drill site.

-barrel tanks, one vapor recovery tower, and one combustor. All equipment appeared to be assembled and ready for use. A 60 feet by 60 feet area was surrounded by caution tape where the release occurred. One lined roll off container was located by the area of the release. Identification was located on the well, on the tank battery, and posted at the entry along with laminated drilling permits.
