0049663816 2019-01-18 Production Wells No Violations The well remains temporarily abandon with two cast iron bridge plugs. Ascent plans to drill out the plugs and install 2.875-inch diameter production tubing and bring the well back into production. Fortis Energy Services had a derrick service rig on location to drill out plugs.

During my inspection the well was equipped with a production tree style wellhead with open valves. Gauges present read 0 pounds per square inch on the 13.375-inch diameter surface casing, 0 pounds per square inch on the 9.625-inch diameter intermediate casing, 1600 pounds per square inch on the 5.5-inch diameter production casing, 900 pounds per square inch on the production tubing. The production equipment on location consisted on seven sand traps, seven gas production units, three horizontal heater treaters, four compressors, six 400-barrel tanks, five 500-barrel tanks, two vapor recovery towers, and one combustor.

0930912668 2018-10-11 Production Wells No Violations During my inspection Patterson 577 was no longer on location and the well was awaiting to be put back into production after being decommissioned for drilling operations on 4 new wells on pad.
1021016748 2019-02-21 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with open valves. The production equipment on location consisted on seven sand traps, seven gas production units, three horizontal heater treaters, four compressors, six 400-barrel tanks, five 500-barrel tanks, two vapor recovery towers, and one combustor. The four recently completed wells on location were flowing back. The flow back equipment on location consisted of three gas buster tanks, one frac tank, and three horizontal line heaters.
1364793328 2018-09-25 Production Wells No Violations The purpose of my inspection was a status check on drilling operations. Accompanied by ODNR Kenn Frye, during our inspection we completed a walk around with Ascent Resources Superintendent James McGee. Patterson 577 was on location in the process of drilling the 10H. This well had been decommissioned prior to rig Patterson 577 moving onto location. The well was equipped with valves on all casing strings. All valves showed 0 pounds per square inch and the a cage was placed over the well.
1416320435 2014-11-16 Casing String Witnessed No Violations On the day of my inspection I found that Nomac Drilling rig 80 had drilled a 17.5 hole to a depth of 979. They had new 13.375 casing in the ground, and were circulating when I arrived. They had to wash the last 40 of casing in. O-Tex Pumping broke circulation and pumped gel spacer followed by cement. Good circulation was maintained throughout the mixing process and there were no significant increases in pressure. They released the plug and were 15.3 barrels into displacement when the well instantly pressured up to 1760 psi. They shut down and let the pressure fall below 1500 psi. They attempted to pump again and the pressure immediately increased again after 0.2 barrels of displacement. O-Tex tried this two more times with the same result. AEU shut the job down and let the cement set up. They drilled the cement out of the casing and had a cement bond log ran. The log showed the top of the cement at 240.
1416336692 2014-11-18 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Nomac Drilling rig 80 ran 1 tubing to a depth of 140 in the annulus of the 13.375 casing. O-Tex Pumping grouted the casing with class H cement. The wellhead had just been made up and pressure tested to 750 psi. They installed the blow out preventer and pressure tested the surface casing at 500 psi. for 30 minutes. There was no decrease in the pressure during the test.
1417019746 2014-11-24 Casing String Witnessed No Violations On the day of my inspection I found that Nomac Drilling rig 80 had drilled a 12.25 hole on air to a depth of 6507. They were in the process of running new 9.625 casing, equipped with a multi-stage tool at 3408. They were having problems getting the last 70 in and had to wash the casing down. O-Tex Pumping established circulation and pumped the first stage of the job. They landed the plug, increased the pressure by 500 psi and held for 5 minutes. Weatherford opened the multi-stage tool and O-Tex pumped to confirm the tool was open. Nomac 80 circulated through the tool for three hours. O-Tex hooked up and pumped the second stage of the job. Good circulation was maintained throughout with approximately 58 barrels of cement circulated to surface.
1449453923 2017-02-07 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a plunger lift system with valves in the open position. Casing pressure was at 650 pounds per square inch and the tubing was at 500 pounds per square inch with annular pressure monitoring in place. The production equipment included 12-400 barrel tanks within containment, 2-NGL tanks, a vapor recovery tower, 2-compressor units, 2-horizontal heater treaters, 2-oil stabilizer units and a gas combustor all shared with the 1H and 5H. Also present a GP Unit and a sand trap with identification present and production records current.
1536101889 2018-09-04 Production Wells No Violations The purpose of my inspection was a status check. The well was not in production while Patterson 577 was on location drilling 34059245250000. I spoke with Ascent Company man and he stated that the well is checked at least one time a day.
1618683193 2018-09-25 Production Wells No Violations The purpose of my inspection was to witness the status of temporary abandonment on well #2-4370. Guernsey County Inspector Austin Gutridge was present during the inspection. While on location we met with Ascent Drilling Superintendent James McGee. At the time of inspection, Patterson #577 was on location drilling on well Red Hill Farm 10H- #2-4526. The drilling crew was making a trip to change the drilling assembly. Well 3H was temporarily abandoned. The production tree was removed and night cap was in place. Annular valve had pressure gauges installed for monitoring. Pressure transducers were installed on the night cap monitoring port to allow for constant monitoring with alarms. A steel cage was located over the well head. A total of three (3) wells were temporarily abandoned. The surface production equipment had been removed. The transfer point was isolated. No simultaneous operations or anti-collision issues were identified during the inspection.

Completed walk around with company men Jim Andrews and Malcom Trahan revealing no violations.
