0342433584 2014-07-07 Drill / Deepen / Reopen No Violations On the day of my inspection following up on Complaint ID 4743, I witnessed that Nomac Drilling Rig #137 was tripping out of the hole with their cores and preparing to trip back in and drill down to 7925 feet and then setting their balanced cement plug from TD to 6600 feet. Construction crews are onsite building a sound barrier wall which resolves complaint. Site inspection revealed no violations.

During my inspection the well was equipped with a plunger lift system with valves in the open position. The production equipment consisted of ten 400 barrel tanks, a vapor recovery tower, two compressor units, two horizontal heater treaters, an oil stabilizer unit, gas production unit, sand trap, and a gas combustor.

Gauges present read 450 pounds per square inch on the casing and 300 pounds per square inch on the tubing. Production equipment included a sand trap, a GP Unit, a line heater, a compressor, a vapor recovery system and 2 disconnected NGL tanks along with 10-500 barrel tanks shared with the 4H and the 6H within containment.
0539815808 2014-07-09 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #137 had reached a vertical td of 7894 feet and Schlumberger was on site logging the hole. Cores were complete and 182 feet of cores were collected. Upon completion of the logging Nomac will pick-up and run 2.875 inch drill pipe and O-Tex cementing will set a balanced Class H cement plug from 7894 feet to 6600 feet. Construction is continuing on the sound barrier wall with completion expected later today.
0928144856 2016-04-07 Production Wells No Violations On the day of my inspection following up Complaint ID 5295, arriving onsite at 0910 and monitored noise levels until 1000 revealing normal levels. Ascent Resources has completed installation of a sound wall on the Northeast side of the well pad with continuing monitoring of the noise level coming from the pad. I placed call to Ascent Resources EHS Supervisor Bob Adams and requested information on the Lake Pad for the last two weeks. Mr. Adams stated that normal production was occurring at the pad during the time in question. I then placed call to Ascent Production Supervisor Darren Rader and he stated that the temporary sound barrier around the combustor was damaged in the wind storm two weeks ago and that he was having the base engineered to prevent future damage before re-installation. Mr. Rader stated that he would try to have it back in place next week. The well was equipped with a plunger lift system and valves in the open position. Gauges present read 900psi on the casing and 650psi on the tubing. Production equipment included a sand trap, a GP Unit, a line heater, an oil stabilizer unit, a compressor, a vapor recovery system, a gas combustor and 2-NGL tanks along with 10-500bbl tanks shared with the 4H and the 6H within containment.

Gauges present read 325 pounds per square inch on the casing and 250 pounds per square inch on the tubing. Annular pressure monitoring was present along with proper identification and production records being current.

Met with Company Man Jude Smith and discussed frac. 38 total stages are planned for the 2H, 39 stages for the 4H and 40 stages for the 6H.
1401717889 2014-06-02 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Precision Rathole Inc. drilled a 36 inch diameter hole to the depth of 100 feet and ran 100 feet of 30 inch 118.65#/ft. conductor pipe. O-Tex Pumping LLC was on site and gained circulation with 2bbls of fresh water, 15bbls of gel and 10bbls of fresh water. O-Tex cemented pipe with 310 sacks of class H cement. Good returns of 8bbls and cement stayed surface.

1444843213 2015-10-14 Production Wells No Violations On the day of my inspection following up on complaint ID 5146, I witnessed production operations were normal with tubing at 800psi and casing at 1200psi. Construction of a sound wall was nearly completed on the east side of the well pad. I placed call to Ascent Resources Senior Manager of Corporate Development and Regulatory Relations Ryan Dean and we discussed the noise of the well pad. Mr. Dean stated that once the wall is completed Ascent will be doing additional monitoring and will adjust the wall with the results. Mr. Dean stated that he was not sure if the combustor ran more than normal last night. I stated that I question the level of noise the compressor was making and Mr. Dean stated that the production superintendent would call me to discuss the events of last night that lead to the complaint. Production Superintendent Darren Rader called and stated that nothing was abnormal about the production last night. Monitoring at the Dolins residence revealed nothing louder than 65dbs and a sound barrier wall is being constructed. Mr. Rader also stated that an additional wall will be added along with barriers around the compressor with additional monitoring to take place upon completion.

During my inspection the well was being prepared to be put back online. The well was uncaged, and the plugs set within the tubing and wellhead were removed. The production equipment was being hauled on to location and arranged on pad. Identification signs and drilling permits were posted at entry.

1574634067 2016-02-22 Production Wells No Violations On the day of my inspection I following up Complaint ID 5254, I met with Lease Operators Brad Eichmiller and Phillip Morrison. Mr. Morrison stated that yesterday while changing out the burner tube on the GP Unit a Navarre Water Truck was onsite carrying glycol. The truck driver had the cam lock cap in place but not locked and the valve was cracked resulting in 75 gallons of glycol being released on the pad covering a 15 feet by 35 feet area. Immediately equipment was dispatched to the site and the glycol was pulled up from the pad and approximately 3 inches of stone was removed and placed in a roll off box. Inspection further revealed that the well was back online with 950psi on the tubing and 1250psi on the casing with new stone arriving and W Pol Contracting completing remediation of spill area. I instructed operator that I would like to be informed of all releases onsite to assure compliance with the laws and rules.
