0353147867 2014-09-22 Completion Testing No Violations On the day of my inspection I witnessed Halliburton was on site and frac stage 2 of 30. Met with Company Man Dominic Herrald and completed walk around revealing no violations. Crews were preparing the 4H for stimulation as the 1H and 2H are completed.
0453336807 2014-06-23 Casing String Witnessed No Violations On the day of my inspection I witnessed Patterson UTI Drilling Rig #257 drilled a 17.5 inch diameter hole to the depth of 359 feet and Franks Casing crew ran 353 feet of 13.375 inch 54.5#/ft. J-55 Surface Casing. Stingray Cementing was on site and gained circulation with 110bbls of fresh water. Stingray Cementing pumped 25bbls of gel, 10bbls of fresh water and cemented casing with 440 sacks of class A cement 15.6ppg, 1.2cuft/sk cement. Displaced cement with 50bbls of fresh water had good cement returns of 3bbls, plug was landed at 172psi and pressured to 672psi for 5 minutes and the floats held.

During my inspection the well was equipped with a plunger lift system with production valves in the open position. Annular pressure monitoring was present with valves reading 0 pounds per square inch on the 13.375-inch diameter surface casing, and 500 pounds per square inch on the 9.625-inch diameter intermediate casing. The production equipment on pad consisted of four vertical separators, four sand traps, four horizontal indirect line heaters, one vertical heater treater, eight 400-barrel tanks, one 50-barrel tank, one vapor recovery tower, one gas combustor, and one flare stack. Identification was located on the wellhead.

1400023434 2014-05-13 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Dylan Pendleton, I witnessed Express Energy Services drilled a 32 inch diameter hole to the depth of 104 feet and ran 110 feet of 26 inch 102.63#/ft conductor pipe. Stingray Cementing was on site and gained circulation with 46bbls of fresh water. Stingray Cementing pumped 15bbls of gel, 10bbls of fresh water and cemented pipe with 236 sacks of class A cement. Good returns of 15bbls and cement stayed surface.
1409147646 2014-08-27 Completion Testing No Violations On the day of my inspection I witnessed that completion rigs was on site and had just perforated the 1st stage from 13847-45, 13808-06, 13768-66, 13829-29 and 13689-87 with 9 shots each for a total of 45 shots. Currently tripping out of the hole to check guns and then rig down to move over to the Ripley 2H. Completed walk around with Company Man Brandon Baptiste revealing no violations.

1985363031 2018-04-19 Production Wells No Violations The purpose of my inspection was to follow up on a fire that occurred on the Northeast side of the pad. The containment under the vertical heater treater ignited and caught fire which eventually spread to the vegetation beside the pad. During my inspection the wells were shut in and contractors were installing new containment under the vertical heater treater. The production equipment consisted of eight 400 barrel tanks, one 50 barrel tank, a vapor recovery tower, two compressors, a vertical heater treater, a gas combustor, back-up flare stack, a horizontal line heater, a vertical separator, and a sand trap.
