1324355479 2017-08-02 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 7 inch diameter casing; 4.5 inch diameter casing, 2.375 inch diameter tubing and a plunger lift system with valves in the open position. Production equipment included a 100 barrel tank, a 30 barrel polymer tank, a vertical separator, a drip and a 1.5 inch Honeywell rotary meter with a chart recorder all within an earthen dike. The well was in production at the time of inspection with proper identification present and the production records were current.

1377275281 2013-08-23 Drill / Deepen / Reopen No Violations On the day of my inspection, I witnessed Universal Well Services break circulation with fresh water on the seven inch surface casing, then displace two hundred and fifty sacks of Gas Block cement with 2% calc and 15% CR-3 back to surface, there was 22 barrel of cement returns, when the pump shut down the cement feel back down hole fifteen feet, I instructed the rig crew to grout the seven inch back to surface. The pit was constructed and lined with ten mill black plastic through the flow ditch and around the well head.
