0070192429 2017-06-07 Production Wells No Violations On the day of my inspection I witnessed that the production equipment included 2-100 barrel tanks, a 30 barrel brine tank, a vertical separator, a drip and a 1.5 inch Honeywell rotary meter with a chart recorder all within an earthen dike. The well was equipped with 7 inch diameter casing, 4.5 inch diameter casing, 2.375 inch diameter tubing, rods and an Alten pumping unit powered by natural gas with valves in the open position on the tubing and 1 of 2 valves open on the casing supplying the pumping unit. Well identification was present and production records were current through 2015.

Well identification was present and the site passed preliminary restoration according ORC 1509.072A.
1190669010 2019-01-28 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with 7-inch diameter surface casing with a casing head, 4.5-inch diameter production casing with a tubing head, 2.375-inch diameter tubing, rods, and a pumping unit powered by natural gas. The production valves were in the open position. The production equipment associated with the well consisted of two 100-barrel tanks, one 1550-gallon poly tank, one vertical separator, one drip, and a sales meter. The sales meter had a 1.5-inch diameter sales line, a Honeywell rotary meter, and a chart recorder. All production equipment was within an earthen dike and the identification was located on the tank. The production records are current with gas production submitted in 2017.
