0587249846 2013-06-25 Drill / Deepen / Reopen No Violations On the day of my inspection, I witnessed Formation Cementing rigging up there connections from the pump truck to the casing head and displaced ninety five sacks of class A cement into 100.85 of 9.625 conductor casing, the pit was constructed and lined with ten mill black plastic through the flow ditch and to the casing.
1372285070 2013-06-25 Casing String Witnessed No Violations On this day of my inspection with Mike Hartshorn witnessed that GNL had ran 100.85 of 9.625 conductor casing and Formation Cementing had set with 95sks of class A cement with 100lbs of gel, 250lbs of accelerator and 40lbs of lcm cotton hull seeds. Had good returns and the pit was constructed and lined.
1372291106 2013-06-26 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Formation Cementing displace one hundred seventy two barrels of class A cement with 17% gel into the seven inch surface casing that had good cement returns and held to the surface.
1618561182 2014-08-18 Preliminary Restoration No Violations On the day of my inspection I witnessed that the production equipment included 2-100bbl tanks, a 30bbl brine tank, a vertical separator, a drip and a 1.5 inch Honeywell rotary meter with a chart recorder all within an earthen dike. The well was equipped with 7 inch casing, 4.5 inch casing, 2.375 inch tubing, rods and a pumping unit powered by natural gas with valves in the open position at the wellhead. Well identification was present and the site passed preliminary restoration according ORC 1509.072A.
