0177592445 2014-08-04 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Intern Austin Gutridge, I witnessed that Francis Brothers had tagged previous plug and it had dropped to 2285 feet. Francis Brothers ran tubing back in the hole to 2248 and Formation Cementing was on site and set a 38 sack Class A cement plug from 2248 feet to 1748 feet. Francis Brothers pulled 16 joints of 2.375 inch tubing and Formation Cementing set a 30 sack Class A plug from 1748 feet to 1358 feet covering the Berea. Francis Brothers pulled tubing and shut well in and will return tomorrow and retag with a wireline service to shoot pipe and complete plugging of said well.

1174545679 2014-08-01 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Justin Smith, I witnessed that Francis Brothers had tagged previous plug at 2290 feet. Ran 2.375 inch tubing back in to 2290 and Formation Cementing set a 500 foot plug from 2290 feet to 1790 feet. Francis Brothers pulled up to set the Berea plug and Formation Cementing set a plug from 1790 feet to 1400 feet covering the perforations. Will return Monday to bond log to determine top of cement and shoot 4.5 inch casing to complete plug job.

1695532692 2014-08-05 Plug / Plug Back No Violations I was onsite today along with Austin Gutridge to witness the plugging of this well. The rig crew tagged the cement first thing in the morning at 1383ft. Eclipse had Timco come in today to shoot off the 4. They were able to find free pipe at 802ft. The 4 was shot off at 802ft. They pulled 1 joint out then set a 200ft plug from 760-560ft. They pulled the 4 up to 210ft and set the surface plug. The plug stayed at the top of the hole. I gave the ok for them to start reclaiming the location.

2111971399 2014-07-31 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Justin Smith, I witnessed that Francis Brothers had pulled the rods and prepared tubing for the first plug. Formation Cementing was on site and set a 500 foot isolation plug from 3485 to 2985 for the slotted 4.5 inch casing. Pulled 17 joints of 2.385 inch tubing up to 2985 and stacked another 500 foot isolation plug from 2985 feet to 2485 feet and pulled tubing and will return tomorrow to tag plug and continue plugging operation.
