0371983176 2014-05-14 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Express Energy Services drilled a 32 inch diameter hole to the depth of 107 feet and ran 110 feet of 26 inch 102.63#/ft conductor pipe. Stingray Cementing was on site and gained circulation with 15bbls of gel, 10bbls of fresh water and cemented pipe with 236 sacks of class A cement. Good returns of 18bbls and cement stayed surface.
0638662797 2017-02-07 Production Wells No Violations On the day of my inspection I witnessed the production equipment included 8-400 barrel tanks and 1-50 barrel tank within containment, a vapor recovery tower, a vapor blow case, a disconnected horizontal 3-phase horizontal, 2-compressor units, a vertical heater treater, a gas combustor and back-up flare stack all shared with the Ripley 2-26H, Ripley 3-26H, and Ripley 4-26H. Also present a horizontal line heater, a vertical separator and a sand trap. The well was equipped with a production tree with valves in the open position with 200 pounds per square inch on the tubing. Annular pressure monitoring was present along with proper identification and production records being current.
0705235958 2016-01-05 Administrative Inspection No Violations At 4:45PM I received a call from EHS Supervisor Brad Burwell stating at 2:30PM condensate reached the stick flare and burned out. Mr. Burwell stated that no product reached the well pad and that it was all flared off. I then received a call from Production Supervisor Robert Coots and discussion was held on what lead up to the incident. Mr. Coots stated that the VRT unit has some type of blockage and has been by passed with vapor leaving the tank and going to the combustor. The VRT unit will be dissembled on Thursday to determine the blockage and upon completion of repairs it will be put back online.

0869670463 2016-01-06 Production Wells No Violations On the day of my inspection following up from the flaring incident that occurred 1/5/16, I witnessed the VRT was bypassed and the remaining vapor recovery system in operation. There were no visible signs of fire or product which reached the well pad. Discussion with Production Supervisor Robert Coots was held and as reported the VRT will be disassembled tomorrow to determine where the blockage is at and repairs will be made to put back online.

Also present a horizontal line heater, a vertical separator and a sand trap. The well was equipped with a plunger lift system with valves in the open position with 250 pounds per square inch on the tubing and 350 pounds per square inch on the casing. Annular pressure monitoring was present along with proper identification and production records being current.
1448376371 2015-11-24 Production Wells No Violations On the day of my inspection I received a call from EHS Supervisor Brad Burwell stating that a flash fire took place this morning near the combustor servicing the pad. No one was injured, the fire was extinguished immediately and no product reached the surface. Upon arrival at 8:39AM I met with EHS Representative Nathan Ray, Production Supervisor Robert Coots and Well Technician Ryan Bates. Mr. Bates stated at 7:12AM he was onsite and saw the fire which was approximately 15 feet and flaring to the southeast near the combustor at that time he hit the EST button shutting in all 4 wells and then he isolated the vapor line and proceeded to use a fire extinguisher and with one pull extinguished the fire. Inspection further revealed that the solenoid for the actuator control valve on the vapor line flowing to the combustor which is regulated with 30psi has a vent which failed in the open allowing a vapor plum to form which found the ignition source at the combustor base. Gulfport will change the current setup to allow the vapors from the solenoid to be re-introduced to the vapor recovery system. All wells will remain shut-in until installation of new equipment and testing has taken place to ensure the vapors will not accumulate in the atmosphere. EMS was not notified as the fire was contained immediately and no product reached the surface of the pad.

1524152385 2018-04-19 Production Wells No Violations The purpose of my inspection was to follow up on a fire that occurred on the Northeast side of the pad. The containment under the vertical heater treater ignited and caught fire which eventually spread to the vegetation beside the pad. During my inspection the wells were shut in and contractors were installing new containment under the vertical heater treater. The production equipment consisted of eight 400 barrel tanks, one 50 barrel tank, a vapor recovery tower, two compressors, a vertical heater treater, a gas combustor, back-up flare stack, a horizontal line heater, a vertical separator, and a sand trap.

1615085016 2014-07-30 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Justin Smith, I witnessed that Patterson UTI Drilling Rig #257 was drilling their curve with an 8.5 inch diameter bit at depth of 7331 feet on 10.5# mud. Completed walk around which revealed no violations.
1651028624 2014-08-12 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Brandon Trigg, I witnessed that the sump valves were open and draining unattended. Met with Justin Cugini of Well Service Group and asked who was watching the sumps as they were being drained and he was unaware that the valves were open. Discharge water was tested with Quantabs which read 130ppm. I asked Justin to shut valves since they were cleaning the rig mats and no one was watching the discharge. Justin then shut and locked the valves in the closed position. I then spoke with Hal Miller of Gulfport and I stated that someone needs present to monitor water while discharge is taking place.

During my inspection the well was equipped with a plunger lift system with production valves in the open position. Annular pressure monitoring was present with valves reading 0 pounds per square inch. The production equipment on pad consisted of four vertical separators, four sand traps, four horizontal indirect line heaters, one vertical heater treater, eight 400-barrel tanks, one 50-barrel tank, one vapor recovery tower, one gas combustor, and one flare stack. Identification was located on the wellhead.
