1379012701 2013-09-12 Casing String Witnessed No Violations On the day of my inspection, I witnessed the rig crew for Monroe Drilling working on the hydraulic pump for the top drive, after the repairs were made the crew ran 1,435 of seven inch surface casing in the 8.75 well bore, the pit was constructed and lined with ten mill black plastic through the flow ditch and around the well head.
1526064376 2017-08-02 Production Wells No Violations On the day of my inspection I witnessed that the production equipment included a 100 barrel tank, a 30 barrel polymer tank, a vertical separator, a drip and a 1.5 inch Honeywell rotary meter with a chart recorder all within an earthen dike. The well was equipped with 7 inch diameter casing; 4.5 inch diameter casing, 2.375 inch diameter tubing and a plunger lift system with valves in the open position. Gauges present read 260 pounds per square inch on the tubing and 420 pounds per square inch on the casing. Identification was present and production records were not current.

The well was equipped with 7 inch casing; 4.5 inch casing, 2.375 inch tubing and a plunger lift system with valves in the open position at the wellhead. Well identification was present and the site passed preliminary restoration according ORC 1509.072A.
