0855682048 2017-02-16 Production Wells No Violations On the day of my inspection I witnessed that the production equipment included a 100 barrel tank, a 37 barrel polymer tank, a vertical separator, a drip and a 1.5 inch Mercury rotary meter with a chart recorder all within an earthen dike. The well was equipped with 7 inch diameter casing, 4.5 inch diameter casing, 1.5 inch diameter tubing, rods and a Jensen pumping unit powered by electricity with valves in the open position. Identification was present and production records were current.
1262799208 2010-01-06 Drill / Deepen / Reopen No Violations Met with company rep to discuss proposed storage tank site, he was inquiring about placing tanks closer to road to keep tanks out of flood prone area, tanks will be located approximately 35 from county road told them that we would grant variance for tank placement.
1675052713 2010-12-10 Preliminary Restoration No Violations Restoration work at site satisfactory pit has been filled in and site reclaimed and vegetation has been established.

Note: Well is equipped with pumping unit and tubing and rods and 2-100 bbl tanks with dike in place.
