0181373153 2019-03-25 Production Wells No Violations I received anonymous call about a smell of gas coming from a tank battery and compressor area along State Route 265 near the intersection of Yoker valley road. I met at the tank battery with M&R Investments well tender, William Flake. While utilizing the Ventis Pro 4 Gas detector zero readings were recorded. The production equipment consisted of one compressor, one 210-barrel tank, one 50-barrel tank, a vertical separator, and a Mercury rotary sales meter with a chart recorder. Mr. Flake stated that it is possible that the smell comes from the vent stacks on the 210-barrel tank. The tank sits roughly 20-feet below the elevation of the road and the vent stacks on the tank are near level to the road. The tank battery is shared between 3405924103, 3405924104, and 3405924105.
0472742865 2017-03-28 Production Wells No Violations On the day of my inspection I witnessed the gas leak that was found on the flowline at Global Positioning System coordinates 39.965511 -81.318801 has been repaired with the well back online. The well was equipped with 7 inch diameter casing, 4.5 inch diameter casing, 1.5 inch diameter tubing, rods and a Churchill pumping unit powered by natural gas with valves in the open position. Identification was present and production records were current. The production equipment included a 210 barrel tank, a 50 barrel tank, a vertical separator, a drip and a 1.5 inch Mercury rotary meter with a chart recorder all within an earthen dike and shared with the 34059241040000 and 34059241050000. 0926232087 2017-02-17 Production Wells No Violations On the day of my inspection I witnessed that the production equipment included a 210 barrel tank, a 50 barrel tank, a vertical separator, a drip and a 1.5 inch Mercury rotary meter with a chart recorder all within an earthen dike and shared with the 34059241030000 and 34059241050000. The well was equipped with 7 inch diameter casing, 4.5 inch diameter casing, 1.5 inch diameter tubing, rods and a Churchill pumping unit powered by natural gas with valves in the open position. Identification was present and production records were current. A gas leak was found on the flowline at Global Positioning System coordinates 39.965511 -81.318801. I placed to well tender Chris Flake who arrived and shut-in the well and unearthed the flowline to find that the line had parted and shifted. Mr. Flake called in his findings and the well will remain shut-in until a backhoe can be brought in and repairs can be made.
1248223523 2010-08-31 Completion Testing No Violations On site to witness second stage of frac job 315,000 gal of water was used and 60 gal of pro slick and 50 gal of pro clay firm and 50 gal of shale slick was used during frac job, average treating pressure was noted at 2650 psi at 45-50 bbl per min.
1282682757 2010-08-24 Completion Testing No Violations On site to witness frac job owner used 15-500 bbl tanks of water and 55 gal of Pro Slick and 43 gal shale slick and 50 gal of Pro Clay Firm,breakdown pressure was 1295 psi and treating pressure was 2600 psi and maximum pressure was noted at 2950 psi and 1050 psi was noted as ISIP and 1024 psi after five minutes,flowback process was started when I left site, will perf upper zones tomorrow.
