0671485691 2019-05-14 Production Wells No Violations The purpose of my inspection was a follow up on complaint 6130 and compliance notice 720932019. The complaint was made regarding an idle well and the concern for well access in the event of an emergency due to the downed trees a crossed the access road. I wrote a compliance notice on 4/18/2019 stating that the well shall be plugged. During my inspection today, the well was equipped with 8.625-inch diameter surface casing with a casing head, 4.5-inch diameter production casing with a tubing head, 1.5-inch diameter tubing with a Demco master valve, and a plunger lift system. The Demco tubing master valve was in the open position and a new logic control box was installed to assist the plunger lift system. The production equipment consisted of one 210-barrel tank, one 50-barrel tank, one horizontal separator, a drip, and a sales meter. The sales meter had a 2-inch diameter sales line, a .75-inch orifice plate, and a chart recorder with chart dated 5/8/2019. All downed trees have been removed from the access road and well appears to be in production. The stated action completed by M&R Investments satisfies the complaint and the compliance notice.
0720932019 2019-04-18 Production Wells Violations Noted The purpose of my inspection was a follow up on complaint 6130. Landowner, Kathy Albertson, met me at the entry of the access road and directed me to the well. The access road had multiple fallen trees laying a crossed the road. The well was equipped with 8.625-inch diameter surface casing with a casing head, 4.5-inch diameter production casing with a tubing head, 1.5-inch diameter tubing, and a plunger lift system. The production valves were in the closed position. Following the inspection, I called Eddie Moss with M&R Investments and left a voicemail stating my findings from the inspection. M&R Investments has submitted reports of zero production for the years 2017 and 2018. M&R Investments shall submit a written plugging schedule to Austin Gutridge at 5880 Memory Rd., Zanesville Ohio 43701 within 30 days, per ORC 1509:13(B)(7) and plug the well within 180 days. M&R Investments will be notified if the plugging schedule is approved or denied.

Production equipment included a 210bbl tank, a 50bbl brine tank, a horizontal separator, a drip and a 2inch sales meter with a .75 inch orifice and a chart recorder all within an earthen dike. Identification was present and production records were current. ODNR never received a plan to address the leak or the erosion from last inspection on March 17, 2015 and current inspection revealed no further attempts were made to address the leak at the wellhead and erosion issues. With the spring rains the erosion has increased making the road is impassible for a vehicle to drive to the well. Owner/operator shall repair wellhead to keep gas from venting and remediate lease road in accordance with ORC 1509.072A.

Readings taken with ODNR Honeywell PhD6 meter revealed no gas at the time of inspection but a light odor was present. I placed call to Pat Ervin and discussed the complaint and Pat stated that they would dress access road again issues and they would come up with a plan to address the leak at the wellhead.
1475678811 2016-10-05 Production Wells No Violations Status check inspection as a response to complaint # 5440 received by the Division on 10/04/2016. Upon inspection of the well head the following equipment was observed-4.5 production casing-1.5 production tubing with the tubing valve open and the casing valve closed and the well set up to be produced by the means of a plunger lift system with electronic controlled flow meter. At the production facility a single 210 barrel capacity tank is present along with a horizontal separator internally housing a gas sales meter with a current sales chart present in the meter. Proper well identification is posted. The landowner filing the complaint (Kathleen Albertson) was contacted on this day in regards to the nature of the complaint filed on the previous day. The landowner indicated she did not witness the complaint but a neighbor indicated they smelled gas for the area of the production facility. The landowner indicated she could only smell gas when she opened the meter box to check the gas chart. The landowner was advised that no gas could be detected leaking from the production facility of well head and the production equipment was high pressure and should not be entered by other than the authorized company agent for safety reasons. The landowner was advised that normal production operations would create some odor down wind when the well cycled. Also noted is a 2nd tank and meter in the production facility owned by a 2nd operator. No violations noticed.
