0294208583 2016-01-28 Production Wells Violations Noted On the day of my inspection following up on NOV-1859561716, I witnessed that the production equipment included 2-100bbl tanks, a horizontal separator, a drip, a 2 inch sales meter with a .25 inch orifice and a chart recorder. Dike repairs have been addressed resolving the original NOV. A 3 foot by 5 foot area of brine contamination was present under the burner tube on the west 100bbl tank. Temporary repairs were made sealing off the tube but a drip was still present at the time of inspection revealing >6078ppm chlorides and pH of 5. The well was equipped with 8.625 inch casing, 4.5 inch casing, 2.375 inch tubing, rods, a Jensen pumping unit powered electrically and a methanol tank within an earthen dike with 1 of 2 valves open on the tubing and 1 of 2 valves open on the casing. Identification was present and production records were current. I placed call to Stonebridge and spoke with Amanda Foster requesting the issue be addressed. Ms. Foster relayed my message to the field supervisor for action. Owner/operator shall remove 100bbl tank, remove contamination, dispose of in a solids landfill and replace with clean fill. Field Supervisor John Schneider called and stated that the tank would be emptied and as soon possible the tank would be removed and area remediated.

0774219014 2016-04-13 Production Wells No Violations On the day of my inspection following up on Compliance Notice-294208583, I witnessed that Stonebridge Operating was onsite and removing contamination and replacing with clean fill resolving notice. Stonebridge will dispose of soil at Hocking-Athens Landfill and provide ODNR with waste record. Also the old tank ladder and fittings were removed and hauled back to their yard.
1457029412 2016-03-03 Production Wells No Violations On the day of my inspection following up on NOV-294208583, I witnessed that the 100bbl tank has been removed and dumb line was plumbed directly into the second 100bbl tank. A backhoe was onsite to remediate inside of dike.

I reviewed The Division of Oil and Gas production records and found them to be current, the proper identification was present at the well and the production tank. An extension was granted until May 16th 2014 to make the repairs to the containment dike due to the ground being frozen when the compliance was issued.

Production records were current and identification was present at the well and the tank battery an extension was granted until May 16, 2014 to make repairs to the dike.

1859561716 2014-02-14 Production Wells Violations Noted On the day of my inspection, I reviewed the Division of Oil and Gas production records and found them to be current for the years of 2011 and 2012; the well is equipped with a pumping unit above an eight and five-eighths inch surface casing with four and a half inch production casing and two inch tubing. Upon inspection, the pumping unit was not operating. The valve on the four and a half inch production casing as well as the two inch tubing at the well was in the open position. The two inch tubing leads to a separator / chart recorder / drip, and (2) one hundred barrel production tanks sitting inside a containment dike. The valve going into the separator was in the open position, and the valve from the separator to the production tanks was in the closed position. Upon inspection, it was observed that the construction of the containment dike was not sufficient to contain one hundred and ten percent of the production tank capacity. The date posted on the chart recorder was February 12, 2014. The identification information posted on the production tanks was not for the current operator of the well.

The requirements to meet the Division of Oil and Gas standards are as follows: 1.) Properly display identification information at the well and/or tank; 2.) Containment dike construction must be repaired to contain the required one hundred and ten percent of the production tank capacity.
