0950585821 2016-07-19 Production Wells No Violations At 13:02 I received a call from Ohio Environmental Protection Agency (OEPA) Emergency Response Chris Holmes stating that OEPA received a call from Heidi Grewell 69935 Leeper Road Cambridge, OH 43725 who stated that there was a leak in which the well owner repaired but oil was still in Wills Creek. Upon arrival at 13:35 I witnessed a 2 inch steel line at Global Positioning System (GPS) coordinates 40.118114 -81.562019. There was no gas leak or oil present in the creek however algae was found. I placed a call to Owner/Operator of Jadco Energy Jim Hagan who arrived at 13:48 and then stated that at 16:00 on 7/18/16 he responded to the leak which was repaired at 16:45 with no oil escaping into the water way. OEPA Emergency Response Chris Holmes and Ohio Department of Natural Resources Division of Wildlife Officer Brad St. Clair arrived at 14:05 and utilizing an absorbent pad it was determined that no oil contamination was present in the creek. The landowner was informed of the findings by Division employees. Inspection of the well revealed the production equipment included a 50 barrel tank, a horizontal separator, a drip, a 2 inch salesmeter with a .75 inch orifice and a chart recorder all within an earthen dike. The well was cycling and equipped with 8.625 inch casing; 4.5 inch casing, 1.5 inch tubing and a plunger lift system with valves in the open position on the tubing and 1 of 2 valves open on the casing supplying the motor valve. Identification was present and production records were current. Downstream of the separator the salesline proceeded west past the said GPS coordinates to a compressor and master meter located on Tyner Road.
