The well was equipped with 8.625 inch diameter casing; 4.5 inch diameter casing, 2.375 inch diameter tubing and a plunger lift system with valves open on the tubing and 1 of 2 valves open on the casing supplying the motor valve. Gauge present read 580 pounds per square inch on the casing. Identification was present and production records were current.

1525798959 2018-05-08 Plug / Plug Back No Violations Red Hill Development contacted me on 5/7/18 and stated that they will be preparing to move a rig on location to begin plug back operations. During my inspection the well was equipped with a plunger lift system with 8.625 inch diameter surface casing with a casing head, 4.5 inch diameter production casing with a tubing head, and 1.5 inch diameter tubing. The 8.626 inch diameter casing had a gauge present reading 375 pounds per square inch and the 4.5 inch diameter casing had a gauge present reading 110 pounds per square inch. A derrick service rig was parked on location but not rigged up.
1527005219 2018-05-16 Plug / Plug Back No Violations The purpose of my inspection was to witness the cementing of the 4.5 inch diameter production casing. The 4.5 inch casing was used weighing 10.5 pounds per foot. During my inspection I witnessed Red Hill run the 4.5 inch casing to 4050 feet within the 7.875 inch diameter borehole. Petroset cementing arrived on location and pumped 110 barrels of fresh water, 10 barrels of gel and 36 barrels of cement. The cement was 50/50 pozmix weighing 14.6 pounds per gallon with a cement yield of 1.29 cubic foot per sack.
1534731641 2018-08-16 Preliminary Restoration No Violations The purpose of my inspection was a follow up on a preliminary restoration. The well was plugged back from the Queenston to the Marcellus. During my inspection the well was equipped with 8.625-inch diameter surface casing with a casing head, 4.5-inch diameter production casing with a tubing head, and 1.5-inch diameter tubing. The production valves were in the open position. A chart recorder was attached to the well reading 15 pounds per square inch on the tubing and 150 pounds per square inch on the production casing. The production equipment within an earthen dike consisted of one 140-barrel tank and one horizontal separator. The frac tanks have been removed from location. The area will need regraded with vegetation established to reach preliminary restoration status.

During my inspection the well was equipped with a plunger lift system with valves in the open position. The well was recently plugged back and recompleted within the Marcellus Formation. The location had been restored with vegetation growth and a gravel lease road. The location meets requirements for preliminary restoration written within the Ohio Revised Code 1509.072 (A). A swabbing rig was on location.

Petroset Cementing arrived on location and set a 30 sack class A cement plug within the 4.5 inch casing from 5776 feet to 5456 feet a crossed the perorations. The cement was squeezed through the perforations and displaced with 14.5 barrels of fresh water.
