1441365182 2015-09-04 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Austin Gutridge, I witnessed Berry Well Service had tripped 1.5 inch tubing to 5493 feet. Formation Cementing Services was on site and cleared tubing and broke circulation with 40bbls of fresh water and circulated wellbore with a total of 120bbls of fresh water. Formation pumped a 31 sack 400 foot Class A Clinton plug from 5493 feet to 5093 feet. Cement was displaced with 12.8bbls of fresh water. BWS tripped 1.5 inch tubing out and will rip 4.5 inch casing on Tuesday.

Cement was displaced with 14.3bbls of fresh water. BWS tripped 2.375 inch tubing up to 1542 feet and Formation Cementing gained circulation and set a 54 sack Class A cement bottom of the surface casing plug with 2% gel from 1542 feet to 1342 feet. Cement was displaced with 5.2bbls of fresh water. BWS tripped 2.375 inch tubing up to 200 feet and Formation Cementing gained circulation and set a 54 sack Class A surface plug with 2% gel from 200 feet to surface. BWS pulled remaining tubing and surface plug was topped off with 17 sacks and cement stayed at the surface. Operator was Instructed to cut casing off 30 inches below grade and install metal plate identifying well and reclaim according to ORC 1509.072B.
