0236848031 2015-10-06 Plug / Plug Back No Violations On the day of my inspection I witnessed Berry Well Service was unable to utilize the 3.875 inch Bulldog Overshot and tripped back with the 675 feet of rods and try to pull more rods. BWS was able to trip out 5025 feet of rods leaving 330 feet of rods and the pump. BWS tried unsuccessfully again to free up 1.5 inch tubing. Decision was made to shoot tubing at 4100 feet and perforate the Big Lime and cement tomorrow.

1388352449 2015-09-30 Plug / Plug Back No Violations I received a phone call from Donnie Strange of Berry Well Service where he stated that they have not been successful unseating the pump or freeing up the tubing. BWS has pulled 34,000 on tubing with stretch it is calculated free point is approximately 4200 feet. BWS pulled stretch on tubing and pumped 50bbls of fresh water down the backside and shut well in until tomorrow.
1444753967 2015-10-13 Plug / Plug Back No Violations On the day of my inspection I witnessed Berry Well Service was unable to shoot 1.5 tubing off due to blockage. BWS torqued on tubing and retrieved 3716 feet of 1.5 inch tubing from wellbore. Appalachian Well Services tripped inside the 4.5 inch casing to perforate and sat down at 2050 feet. BWS tripped 2.875 inch tubing and sat down at 2360 feet. BWS worked 2.875 inch tubing and was able to get to a depth of 3763 feet. Appalachian Well Services was dispatched back to site and tripped inside the 2.875 inch tubing to a depth of 2050 feet and sat down again. It was determined that the 1.5 inch tubing was sitting at that depth with the 2.875 inch tubing over it. BWS will trip 2.875 inch tubing from wellbore and will trip back in 3.875 inch overshot with a grapple on 2.875 inch tubing to attempt to fish tubing from wellbore.
1445254428 2015-10-15 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Doug Bennett, I witnessed Berry Well Service had tripped inside the 4.5 inch casing with 2.875 inch tubing and 3.875 inch overshot with a grapple and sat down at 3850 feet. BWS pulled and rotated tubing resulting in a 6000psi gain in string weight. BWS tripped 2.875 inch tubing out and retrieved the remaining 1.5 inch tubing, rods and a packer. Appalachian Well Service was on site and tripped inside the 4.5 inch casing to a depth of 5414 feet verifying the perforations were at the correct depths from 5382 feet to 5355 feet. AWS tripped up to a depth of 3882 feet and successfully perforated the Big Lime with 10 shots from 3900 feet to 3910 feet. AWS tripped out the 4.5 inch casing and rigged down. BWS began tripping in 2.875 inch tubing inside the 4.5 inch casing and will be cementing the bottom plug tomorrow.
1445353908 2015-10-20 Plug / Plug Back No Violations On the day of my inspection I witnessed Berry Well Service had tripped 1.5 inch tubing in to a depth of 2970 feet. Cemenco Services Inc. was on site and gained circulation and pumped a 30 sack 100 foot Class A rip plug from 2970 feet to 2870 feet. Cement was displaced with 7.2bbls of water. BWS tripped 1.5 inch tubing to 1590 feet. Cemenco gained circulation and pumped a 60 sack 200 foot Class A bottom of the surface plug from 1590 feet to 1390 feet. Cement was displaced with 3.5bbls of fresh water. BWS tripped 1.5 inch tubing to 200 feet and Cemenco gained circulation and pumped a 60 sack 200 foot Class A surface plug from 200 feet to surface. BWS tripped remaining 1.5 inch tubing and cement stayed at the surface.
1759835256 2015-10-16 Plug / Plug Back No Violations On the day of my inspection I witnessed Berry Well Service had tripped 2.875 inch tubing to 5395 feet and Cemenco was on site and pumped a 400 foot 30 sack Class A Clinton plug from 5356 feet to 4956 feet. Cement was displaced with 28.6bbls of fresh water. BWS tripped 2.875 inch tubing to 3910 feet and Cemenco pumped a 60 sack Class A Big Lime plug from 3910 feet to 3710 feet. Cement was displaced with 21.4bbls of fresh water. BWS tripped remaining tubing from wellbore and will rip 4.5 inch casing on Monday.

1818405710 2015-10-19 Plug / Plug Back No Violations On the day of my inspection I witnessed Berry Well Service had tripped in the wellbore and tagged up at 3400 feet. BWS gained telegraph and successfully ripped the 4.5 inch casing at 2978 feet and tripped out casing. BWS will run 1.5 inch tubing back in and plug well out tomorrow.

BWS got the rods to break at 675 feet and tripped from the well. BWS began working tubing with Berry Well Service Vice President of Field Operations Bill Mitton on site. BWS pulled 33,000 stretch and calculations were made the free point is approximately 4200 feet. BWS continued to work tubing and Mr. Mitton stated that they would be bringing out 2.875 inch tubing and obtaining a 3.875 inch overshot to trip inside the 4.5 inch casing to retrieve the 1.5 inch tubing.
