Production valves were in the closed position. A domestic gas service was attached to the 4.5-inch casing with a regulator, meter, and an open valve. The production equipment consisted of one 210-barrel tank, one horizontal separator, one drip, and a sales meter. The sales meter had a 2-inch diameter sales line, a .75-inch orifice plate, and a chart recorder with current chart. The production records were current. The identification was located on the tank.

1527693754 2018-05-29 Production Wells No Violations One 210 barrel tank and one horizontal separator are placed within an earthen dike. Inspection found 45 feet by 8 feet area of stressed vegetation on the downhill slope of the west side of the dike. The source of the stressed vegetation was a 4 inch PVC dike drain that was located in the base of the dike. A layer of produced fluids surrounding the 210 barrel tank roughly 2 inches deep was lying in the bottom of the dike that makes up a 35 feet by 18 feet area. The 210 barrel tank was leaking through the valve on the sales outlet connection. I tested the fluid within the dike with high range Quantab titrators for Chloride and the results exceeded the maximum readable limit of 6298 parts per million. I called Tim McConaughey with Enervest operating and he stated that he would send someone out as soon as possible. Enervest well tender Rick Spratt arrived on location and evaluated the area. Mr. Spratt called a water truck and the roustabout crew to assist in clean-up operations. A water truck arrived and removed the fluids from within the dike and placed a solid plug within the sales outlet connection on the tank and prevented the tank from leaking. A roustabout crew will be here tomorrow to remove the contaminated soil, seed and mulch the area.
1876612685 2018-05-30 Production Wells No Violations The purpose of my inspection was to witness clean-up operations. Enervest Operating roustabout crew removed the dike drain, rebuilt the dike wall, removed the contaminated soil, and seeded and mulched the area where the soil was removed. I evaluated the tank and found that it was not leaking.
