Records indicate well was plugged but it does exist and photos were taken of well.

Plug well or file for temporary inactive status. Failure to comply may result in bond forfeiture. Contacted central office to get status of well changed from FR to pr. And belongs to Enervest Operating.

0525702256 2015-05-27 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Austin Gutridge, I witnessed Berry Well Service had tripped 1.5 inch tubing to 3400 feet. Cemenco was on site and pumped a 150 foot 45 sack Class A Rip plug from 3340 feet to 3190 feet. BWS pulled 53 joints of 1.5 inch tubing and Cemenco pumped a 200 foot 60 sack Class A bottom of the surface plug from 1546 feet to 1346 feet. BWS pulled 42 joints of 1.5 inch tubing and Cemenco pumped a 90 sack 280 foot Class A cement Surface plug from 280 feet to surface. BWS pulled remaining 9 joints of 1.5 inch tubing and cement stayed at surface.
1075998646 2015-05-21 Plug / Plug Back No Violations On the day of my inspection I witnessed Berry Well Service had pulled the 1.5 inch tubing from wellbore and retrieved the plunger. Appalachian Well Service perforated the Big Lime with 10 Shots from 4060 feet to 4050 through the 4.5 inch casing. BWS began running 1.5 inch tubing back in the wellbore to bottom to commence plugging. Cemenco was onsite and pumped a 400 foot 30 sack Class A bottom hole cement plug from 5596 feet to 5196 feet. BWS pulled 47 joints of 1.5 inch tubing and Cemenco pumped a 200 foot 60 sack Class A Big Lime cement plug from 4065 feet to 3865 feet. BWS will pull remainder of the tubing and rip the 4.5 inch casing

1371138383 2013-05-15 Production Wells Violations Noted On the day of my inspection I reviewed The Division of Oil and Gas production records and found them not current, the last recorded production was the year of 2003. My inspection at the well revealed the well head was painted yellow, my gas dector did not record any H2S at the time of my inspection, there is seven inch casing with inch and a half tubing with a gate valve standing from the ground, there is inch and a half steel production line and valve leading from the well into a separator/ drip and (1) two hundred ten barrel production tank setting inside a containment dike, all the valves were closed at the well head and the I.D. is posted. The requirements to meet the Division of Oil and Gas Standards will be to plug the well.
1373051376 2013-09-30 Production Wells No Violations On the day of my inspection, I found there to be no change in the status of the well since my last inspection, I spoke with Exco - North Coast about the compliance notice that was sent on 05/15/2013, Exco - North Coast informed me they filed the form seven and was going to follow up with Enervest.
1387552735 2013-12-19 Production Wells No Violations On the day of my compliance follow-up inspection, I found the well shut in and painted yellow, my Sperian gas dector did not gather any reading of H2S gas, the well is equipped with seven inch surface casing and inch and a half tubing, the tubing is equipped with a gate valve that is closed, I placed a phone call to Annmarie OConnor ( Paralegal for EXCO Resources ) to discuss the matter of ownership to the well. I received an e-mail from Annmarie OConnor stating, pursuant to our telephone conversation of December 19th, attached please find a copy of the recorded assignment wherein EXCO assigned the Smith J. #2 well (34059228110000) to Enervest. I forward this document to Ed Haas at Enervest and will work with him to get a form seven filed with the Ohio Department of Natural Resources, I then placed a phone call to the landowners attorney Matt Warnock and updated him of the situation with the well.
1392580341 2015-05-26 Plug / Plug Back No Violations On the day of my inspection I witnessed Berry Well Service had ripped the 4.5 inch casing at 3500 feet and was pulling casing from wellbore. Upon completion BWS will tip 1.5 inch tubing back in and plug out well. The production equipment has been removed and the dike has been restored.
1413295124 2014-10-14 Production Wells No Violations On the day of my inspection following up on Complaint ID #4831, I witnessed that the well was not plugged and was equipped with 8.625 inch casing; 4.5 inch casing, 1.5 inch tubing and a plunger lift system with all valves in the closed position. The production equipment included a 210bbl tank, a horizontal separator, a 2 inch sales meter with a .75 inch orifice, no chart recorder was present and valves were closed on the salesline. Division records reflect the well was plugged and status is final restoration. Land owner states that the said well was supposed to be plugged and has H2S on it. No H2S was detected on the Division Honeywell PhD6 gas detector. Photographs were taken along with updated handheld GPS coordinates. Identification was present at the well and the tank with no current production records.
