0419395791 2017-01-24 Production Wells No Violations On the day of my inspection I witnessed the production equipment included a 100 barrel tank, a horizontal separator, a drip and a 2 inch salesmeter with a 1 inch orifice all within an earthen dike. The well was equipped with 4.5 inch diameter casing, 1.5 inch diameter tubing, a plunger lift system and domestic gas service with valves closed on the flowline, open on the tubing and 1 of 2 open on the casing supplying the domestic gas service. Identification was present and production records were current with zero production reported in 2014 and 2015.

1572048415 2014-10-23 Production Wells No Violations I am accompanied today by supervisors Brent Bear and Dan Goins as well as Assistant Chief Scott Kell and Dan Arthur of All Consulting. My inspection finds the tank battery consist of one 100 barrel tank and a horizontal separator. There is no meter present in the tank battery. The identification is very faded, but still legible. I contacted the well tender William Flake and told him to repaint the identification tag. The well is equipped to be produced through the tubing. A domestic gas meter is set at the wellhead.

2081856488 2013-04-24 Production Wells No Violations On the day of my inspection I reviewed the Division of Oil and Gas production records and found them to be current, my inspection at the well site reveled 4.5 production casing with 1.5 tubing and a gate valve standing out of the ground, the production line is closed at the well, there is a plastic 1 domestic gas line leading from the well, there is a separator and (1) one hundred barrel production tank setting inside the containment dike, the I.D. is posted on the tank. There is some contamination inside the dike around the tank, I placed a phone call to the pumper William K. Flake to take of the spill, the valve is open on the tubing.
