0787486041 2013-04-25 Production Wells Violations Noted Pryor to my inspection I received a phone call from the land owner requesting a inspection on the well sitting on his land that has not shown production for the last two years. I reviewed the Division of Oil and Gas production records and found they are not current, the well was being produced through the four inch with a (4x2) swedge and plastic gas line tying into a two inch steel line leading into the separator, the well was venting gas around the four and a half inch collar and the swedge, there is fifty barrel plastic brine tank setting beside the well with a small kill area in front of the valve that measures three foot wide and five foot long. There is (1) two hundred ten barrel production tank with a hole in the side of it that measures seven inches wide by twelve inches long, I found the tank had fluid in it. I spoke with Bill Arnholt on the phone and meet with Northwood Energys pumper to discuss the issues with the well. I told the pumper to bring a water truck in and remove the fluid from the tank on this day 04/25/2013 and remove the tank from the location by 04/26/2013, the pumper is going to make the necessary repairs to the well for the venting of the gas. The requirements to meet the Division of Oil and Gas standards will be to make the well capable of commercial production within thirty days or plugging is required sixty days.

1288493048 2013-06-04 Plug / Plug Back No Violations On the day of my inspection I witnessed Buxton Well Services pull all the 4.5 casing out of the well and run 2 tubing back down to 1487, then Formation Cementing displaced a 300 plug at the bottom of the 8.625 casing which was through the Berea, then pulled thirty nine joints of 2 tubing and checked the fluid level, then pulled the last seven joints and set a 200 plug to surface.

1370362163 2013-06-03 Plug / Plug Back No Violations On the day of my inspection I witnessed Formation Cementing make an attempt to displace a three hundred foot plug at the bottom of the 4.5 casing that was set at a depth of three thousand foot, displacement was set with forty seven barrel of fresh water, then ten joints of the 4.5 casing was pulled so the casing was not setting inside the plug, then Buxton Well Services packed the well off, placed a four to two swedge and valve on top of the 4.5 casing with the valve closed for the nite.
