0397692517 2019-02-26 Final Restoration No Violations The purpose of my inspection was a status check on a final restoration. During my inspection all debris have been removed from location and no evidence of an oil and gas well existed. The location was graded with spotty vegetation growth. A follow up inspection will be conducted this spring to check on the vegetation growth.

During my inspection the location had been graded and spotty vegetation growth had taken place. An electrical panel and wiring had been left on the edge of the wellsite. I witnessed plugging operations on 10/2/18. A follow up inspection will be conducted to insure that the location has met final restoration writtin in 1509.072 (B).

PAPERWORK SENT TO COLUMBUS SOMEWHERE AROUND 1986-87.HE STATED TO ME HE HAS ALL NECESSARY PAPERWORK IN TO THE COLUMBUS OFFICE, AND HAS BEEN USING THIS WELL FOR DOMESTIC SEVRAL YEARS NOW
1455815284 2016-02-18 Production Wells No Violations On the day of my inspection I witnessed the production equipment included a 100bbl tank and 30bbl polymer tank within an earthen dike. The well was equipped with 8.625 inch casing, 4.5 inch casing, 1.5 inch tubing, rods, a Churchill pumping unit powered by electricity and domestic gas service with the valve open on the tubing, casing valve closed on the domestic gas and open on the flowline. The electric power was in the off position. Identification was present but not legible and production records were current but zero oil and gas production reported for 2011, 2012, 2013 and 2014. I placed call to Jamie Cameron and advised I needed to discuss the status of the well. Mr. Cameron stated that he would be applying for a plug permit and plugging said well this summer.
1884314543 2018-10-02 Plug / Plug Back No Violations The well was drilled and completed with 14 perforations from 5252 feet to 5330 feet within the Clinton formation. Chipco was on location with cement trucks and the derrick service rig. 1.5-inch diameter tubing was at 5340 feet and Chipco pumped a 400-foot Clinton plug from 5340 feet to 4940 feet with 30 sacks of class A cement. The plug was displaced with 12 barrels of fresh water. The tubing was tripped from hole and the jar and stem ripping tool was tripped within the 4.5-inch diameter production casing to attempt to establish telegraph and find a spot to rip and remove the casing. Telegraph was achieved at 3817 feet and one 44-foot joint was pulled out of hole to verify that the casing was free. The bottom of the 4.5-inch casing was at 3773 feet and Chipco pumped a 60-sack class A cement 200-foot big lime plug within the 7.875-inch diameter open hole. The 4.5-inch casing was tripped up to 1750 feet and Chipco pumped 60 sacks of class A cement 100 foot within the bottom of the surface casing and 100 foot out within the 7.875-inch open hole. The 4.5-inch diameter casing was tripped up to 150 feet. Chipco pumped a 150-foot surface plug with 45 sacks of Class A cement. The cement stayed at surface.
