0239395144 2018-06-06 Plug / Plug Back No Violations During my inspection Jeff Miley had rigged up on location with a single pole service rig. Rods and tubing were tripped from the well in preparation for plugging operations.
0425947057 2016-03-07 Production Wells No Violations On the day of my inspection I witnessed the well was equipped with 6.625 inch casing, 2 inch tubing, rods and a gasoline powered Alten pumping unit with valves in the closed position. Production equipment included 2-100bbl tanks shared with 20711, 20602 and 22159 within an earthen dike. Jeff Miley of Miley Gas Co. claims he is the owner of the said well and is trying to get it transferred into his name so that it can be plugged.
0497399310 2016-03-29 Production Wells No Violations On the day of my inspection I met with Muskingum County Watershed District Representatives Mark Swiger, John Watkins and Brad Janssen along with Miley Gas Co. Owner Jeff Miley to discuss upcoming plugging of well, pit placement and erosion control measures for the location.
0625001174 2018-06-07 Plug / Plug Back No Violations The well was drilled to the total depth of 640 feet. Well records indicate that the well is equipped with 5.5 inch diameter production casing and 7.5 inch diameter surface casing. Jeff Miley was rigged up on location with a single pole service rig. The rods and tubing were tripped from hole. 2 inch diameter tubing was tripped back within the well to 650 feet and Cemenco set a plug from 650 to 450 feet with class A cement. 1.7 barrels were used for displacement. The well was circulated prior to setting the plug.
0934305719 2018-06-18 Plug / Plug Back No Violations The 2 inch tubing was tripped back in the 6.625 inch diameter casing to 360 feet and Cemenco cementing pumped 85 sacks of class A cement. The cement circulated to surface and stayed at surface.
0978039021 2018-07-12 Final Restoration No Violations The purpose of my inspection was a final restoration. During my inspection all restoration work has been completed. The pit has been closed and the location has been graded, seeded, and mulched with vegetation growth. Two 100-barrel production tanks remain. Once the tanks have been removed the location will meet the final restoration requirements.
1024556129 2018-06-08 Plug / Plug Back No Violations The tubing was tripped from hole and Appalachian Well Surveys arrived on location to shoot the 5.5 inch diameter production casing off at 386 feet. A casing cutter was lowered into the well and successfully shot off at 386 feet. After the casing cutter was shot off the wireline was tripped out of hole. Miley disassembled the wellhead and attempted to pull the 5.5 inch diameter casing with 5.5 inch diameter elevators. While disassembling the wellhead Appalachian and Miley measured the casing and found that the casing that was shot with the casing cutter was not 5.5 inch diameter casing it was 6.625 inch diameter casing. Records did not indicate 6.625 inch diameter casing so at that point Miley dug around the wellhead to try and discover any other casing strings that could possible be in the well. Miley did not discover any casing other than the 6.625 inch diameter casing in the well. I discussed a plan with Miley and John Hudson with Appalachian and we decided not to remove the only casing string in the well and to shoot perforations within the 6.625 inch diameter casing with four shoots each at 360 feet 220 feet and 100 feet. After discussing the situation Appalachian shot four perforations within the 6.625 inch diameter surface casing at 360 feet, 220 feet, and 100 feet.
1216693991 2018-10-03 Final Restoration No Violations The purpose of my inspection was a status check on a final restoration During my inspection thick vegetation was in place and all production equipment has been removed. The location meets final restoration requirements written within the Ohio Revised Code 1509.072(B).
1573849973 2016-03-01 Administrative Inspection No Violations I met with Ruth Burns at her home on route 566 to discuss the well. Ms. Burns stated that she believed the well was sold to Jeff Miley of Miley Gas Company. I contacted Mr. Miley and he indicated that he did own the well and he is in the process of registering the well in his name. Mr. Miley also stated that he intends to plug the well this summer.
