The well was capable of production and equipped with 4.5 inch casing, 2.875 inch tubing, 1.5 inch tubing, rods and an electrically powered pumping unit with 2 of 4 valves open on the tubing and casing. Production records were current and identification was present at the well and the tank and has been corrected. The 210bbl tank remains on site and pumping unit power is in off position.

Called Greg Henderson of Big Sky and requested an update on the status of the well. Greg Henderson returned my call and stated that the tank was empty and will be removed and replaced next week.

During my inspection I witnessed Greg Henderson from Big Sky shut in the domestic gas line. All valves on the well are now in the closed position. Greg Henderson and I went to all homes that were receiving natural gas from this well to inform them that they will be without natural gas.

The production equipment consisted of one 210-barrel tank, one horizontal separator, and a sales meter. The sales meter had a 2-inch diameter sales line, a .1875-inch orifice plate, and a chart recorder with chart dated 4/12/16.

1815635599 2014-02-25 Production Wells Violations Noted On the day of my inspection accompanied by ODNR Inspector LJ Parkins, I witnessed that the tank battery consisted of a 210bbl tank which upon arrival had a hole in it approximately 10 feet up with fluid escaping from tank and leaching on the ground to a 6 foot by 20 foot area. The dike was not capable of containing the spill. Also included in the dike was a horizontal separator, a 2 inch sales meter with a .1875 inch orifice and a chart recorder and domestic gas service. Gas was flowing through meter at the time of inspection. The well was capable of production and equipped with 4.5 inch casing, 2.875 inch tubing, 1.5 inch tubing, rods and an electrically powered pumping unit with 2 of 4 valves open on the tubing and casing. Production records were current and identification was present at the well and the tank battery however the company and phone numbers were incorrect. Made call to Big Sky Energy asked to speak to Bob Barr and the receptionist said he was not there. I stated I was on the Martin #2 well and that the 210bbl tank was leaking and needed it addressed immediately. She stated that she would send someone out. Pumper Greg Henderson for Big Sky arrived and I told him that the tank had to be drained down, removed and replaced and the area had to be cleaned up. Also that the well needed to be shut--R wells were open and he stated that all of Big Sky Energy wells were open. I stated again that the wells needed to be shut-in due to the suspension order from the Chief #2014-43. He stated he was not shutting anything in until Bob Barr told him to. Pumping unit has been shut down and tank has been drained down to oil and awaiting oil truck so tank can be removed and replaced. Contaminated soil has been removed and dike has been built up to meet SPCC standards. Owner shall remove and replace tank and correct identification.
