1283782618 2014-08-11 Final Restoration No Violations On the day of my inspection I witnessed that the tank ladder was still present and the site did not pass final restoration according to ORC 1509.072B. I made call to Enervest and spoke with Troy Wells about ladder on site and Troy stated he would send someone down to pick up ladder.
1387394627 2013-12-18 Production Wells No Violations Pryor to my inspection, I received a phone call from Rocky King (Administrator) for the South Eastern Division of Oil and Gas of a idle well with H2S gas complaint, my inspection revealed no readings of H2S gas was present with my Sperian gas dector, Floating Inspector Doug Bennett was present at the well site with his Sperian gas dector, the well appears to be capable of production and is equipped with eight inch surface casing, four and a half inch production casing with inch and a half tubing, the tubing is equipped with a gate valve that is closed, there is a two inch production line leading from the well into a separator / drip / chart recorder and (1) two hundred ten barrel production tank setting inside a containment dike, the valve on the two inch production line at the separator is closed, there are H2S gas warning signs posted at the well head and at the chart recorder. I spoke with the well tender Dave Shaffer as to why the well was shut in, Mr. Shaffer informed me that in the year of 2012 Enervest Operating hired a service rig to swab the well and at that point the H2S gas was present, the rig crew then shut the well in, I placed a phone call to Tim McConahey with Enervest Operating to see if the well was on there list to be plugged, Mr. McConahey informed me the well tender was going to the well site and check the pressure at the well head and check the parts per million of H2S gas with a monitor so a decision can be made to plug or produce the well, The Division of Oil and Gas production records for the years of 2011 and 2012 are current, the I.D. is located at the well head and on the production tank.
1396549460 2014-04-01 Plug / Plug Back No Violations On the day of my inspection, I witnessed Chipco LLC break circulation with fresh water then displace thirty sacks of Class A cement into the Clinton formation from 5600 up to 5200, then the operator pulled twenty nine joints of the four and a half inch production casing, from that point Chipcc LLC broke circulation with fresh water and displaced sixty sacks of Class A cement in to the Big Lime formation 3970 up to 3770. I then instructed the operator to tag the plug on the following day to insure the Lime plug was still in place. The pit was constructed and lined with ten mill black plastic through the flow ditch and around the well.
1600101771 2013-12-18 Production Wells No Violations On the day of my inspection with ODNR Guernsey County Inspector Mike Hartshorn on a H2S complaint I witnessed that the well was shut in at the tank battery which consisted of a 210bbl tank, a horizontal separator, a sales meter with a .25 inch orifice and identification was in place. The well was equipped with 8.625 inch casing, 4.5 inch casing, and 1.5 inch tubing with a rabbit catcher. The well was shut in, identified and signage was in place for H2S warnings at wellhead. Production records were up to date and ODNR Honeywell PhD6 meter showed zero H2S present at the time of inspection.
