0077826682 2015-12-17 Production Wells No Violations On the day of my inspection following up on NOV-131732295, I witnessed the production equipment included a 100bbl tank, a horizontal separator and a drip all within an earthen dike. The salesline was disconnected on the downside of the drip. The well was equipped with 8.625 inch casing; 4.5 inch casing, 1.5 inch tubing and a plunger lift system with all valves in the closed position. Identification was present and reported production totals were zero for the years 2011, 2012, 2013 and 2014. A plug permit was issued on 10/19/15 with no change in status of well. Utilizing an ODNR Honeywell PhD6 meter no H2S was detected.

I placed call to Enervest Superintendent Tim McConaha where he stated the plan is to commence plugging of the well after the Toland #1 is completed if the weather cooperates.

1464261367 2016-05-26 Production Wells No Violations On the day of my inspection a safety meeting was held with Enervest Superintendent Tim McConaha, Chipco employees and Appalachian Well Surveys. Chipco arrived on location 5/24/16 and found H2S readings at 30ppm and loaded the 4.5 inch casing with fresh water and were unsuccessful at freeing up the 1.5 inch tubing. Chipco tripped inside the 1.5 inch tubing reaching a depth of 4800 feet. Chipco tripped out of the tubing and shut well in. On 5/25/16 Chipco was unsuccessful at freeing up tubing again. Chipco then tripped inside the 1.5 inch tubing with their tools reaching 4800 feet. Chipco tripped out and loaded the tubing with fresh water and shut the well in. On this day H2S readings were at 0ppm and AWS tripped inside the 1.5 inch tubing setting down at 5078 feet. Decision was made to shoot tubing off at 5068 feet with AWS tripping out. Chipco tripped out the 1.5 inch tubing and AWS perforated the 4.5 inch casing with 10 shots from 5038 feet to 5028 feet and then perforated the Big from 4100 feet to 4090 feet. Chipco tripped 1.5 inch tubing in to a depth of 5038 feet and shut the well in until Tuesday.
1464365876 2016-05-26 Plug / Plug Back No Violations The purpose of my inspection today is to witness the plugging of this well owned by Enervest. I was accompanied by ODNR Inspector Doug Bennett. Companies on site during my inspection include Chipco, Enervest, and Appalachian Well Surveys. This well contains H2S but we did not pick up any readings on the gas detector during inspection. See Inspector Doug Bennets report for further plugging details.
1464698737 2016-05-31 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Austin Gutridge a safety meeting was held with Enervest Superintendent Tim McConaha and Chipco employees. H2S readings were at 0ppm and Chipco pumped a 60sack 200 foot Class A cement plug from 5038 feet to 4838 feet and displaced cement with 12.2bbls of fresh water. Chipco pulled 28 joints of 1.5 inch tubing and pumped a 60sack 200 foot Class A cement plug from 4100 feet to 3900 feet and displaced cement with 9.8bbls of fresh water. Chipco tripped 1.5 inch tubing out and tripped inside the 4.5 inch casing attempting to rip casing at 3000 feet, 2968 feet, 2900 feet, 2840 feet and 2764 feet. Chipco ripped casing at 2636 feet, repacked the wellhead and shut in the well until tomorrow.
1464867185 2016-06-02 Plug / Plug Back No Violations On the day of my inspection I witnessed that the cement remained at the surface and the pit has been closed.
1737512158 2016-06-01 Plug / Plug Back No Violations On the day of my inspection I witnessed H2S readings were are 0ppm. Chipco tripped out 4.5 inch casing retrieving 2636 feet and tripped in 1.5 inch tubing to 1536 feet. Chipco pumped a 75 sack 250 foot Class A cement plug from 1536 feet to 1286 feet and displaced cement with 3.2bbls of fresh water. Chipco tripped tubing to 200 feet and set a 200 foot Class A cement plug from 200 foot to surface circulating cement to surface. Chipco tripped tubing out and topped off cement with cement staying at the surface. Instructed operator to cut casing off 30 inches below grade and install metal plate identifying well and reclaim according to ORC 1509.072B.
1802019995 2016-05-31 Plug / Plug Back No Violations The purpose of my inspection today is to witness Chipco plug Enervests H2S well. During my inspection Chipco set the bottom hole plug and the Big Lime plug. Chipco removed the 1.5 inch tubing from the hole and proceeded down hole to rip the 4.5 inch casing. I was accompanied by ODNR Inspector Doug Bennett during the duration of this inspection. See Doug Bennetts report for further plugging details.
