0344137794 2016-08-22 Production Wells No Violations On the day of my inspection I witnessed the contaminated soils and debris had been removed and the area was restored to grade. Enervest will remove the oil booms from the creek tomorrow.
0894936978 2014-06-25 Production Wells No Violations On the day of my inspection I witnessed that the production equipment included a 50bbl tank, a horizontal separator, a drip, a 2 inch sales meter with a .75 inch orifice and a chart recorder. The well was equipped with 8.625 inch casing, 4.5 inch casing, 2.375 inch tubing and a plunger lift system with the valves open on the tubing and 1 of 2 valves open on the casing supplying gas to the motor valve. Proper identification was in place with production records up to date.
1388650742 2016-08-16 Production Wells No Violations At 13:23 I received a call from ODNR North Regional Emergency Operations Manager Terry Whitehair stating that a drip on a salesline had been hit by a landowner mowing a field resulting in a release of fluid in the field and reaching a nearby creek. Upon arrival at 13:50 accompanied by ODNR Customer Service Assistant Jimmy Wiles, I witnessed a 60 feet by 100 feet area around the drip and approximately fifteen thirty foot trees had been misted with oil from the drip along with mist reaching a tributary of Brushy Fork Creek. Enervest Roustabout Terry Miller repaired the drip, placed absorbent booms in two sections of the creek along with absorbent material within the affected area. The trees were determined by Enervest to be within their right away. Enervest stated that the trees would be removed along with the affected soils and disposed of properly. Due to rain at 18:00 remediation was stopped with the trees being removed and the contaminated soil stockpiled for disposal tomorrow.

1906277330 2016-08-17 Production Wells No Violations On the day of my inspection accompanied by ODNR Customer Service Assistant Jimmy Wiles, I witnessed that the production equipment included a 50 barrel tank, a horizontal separator, a drip, a 2 inch sales meter with a .75 inch orifice and a chart recorder all within an earthen dike. The well was equipped with 8.625 inch casing; 4.5 inch casing, 2.375 inch tubing and a plunger lift system with the valves in the closed position. Proper identification was in place and the production records were current. Further inspection of the line which was damaged on 8/15/16 revealed Sutton Pump and Supply representatives were loading contaminated soils and trees for disposal with remediation to be completed today.
