0635402416 2015-05-19 Production Wells No Violations On the day of my inspection I witnessed the production equipment included a 100bbl tank within an earthen dike. Also present was a drip with the well equipped with 8.625 inch casing and 4.5 inch casing reduced to a 2 inch flow line with valves in the closed position. Identification was present and production records were current.
0969925604 2017-06-19 Production Wells No Violations On the day of my inspection I had a meeting with Luke Mallet of Mallet Energy and reviewed his plugging proposal with operations commencing tomorrow.
1148390844 2017-06-28 Plug / Plug Back No Violations On the day of my inspection I witnessed RB Drilling (RBD) was onsite and tripped inside the 4.5 inch diameter casing with their ripper and tagged the bottom hole plug at 5147 feet. RBD tripped up to 4200 and could not gain telegraph and successfully gained telegraph and ripped casing at 4094 feet. RBD tripped out of the casing and laid down 3 joints of 4.5 inch diameter casing leaving it set at 3968 feet. Utilizing an ODNR Rental Ventis meter revealed Hydrogen Sulfide (H2S) levels at 0. Also utilizing ODNR Tango H2S detector revealed H2S levels at 0. Formation Cementing Inc. arrived and pumped a 38 sack 150 foot Class A Big Lime plug from 3968 feet to 3818 feet. Cement was displaced with 60.8 barrels of brine water. RBD tripped 59 joints of 4.5 inch diameter casing leaving it set at 1484 feet. Formation Cementing Inc. gained circulation and pumped a 54 sack 200 foot Class A Bottom of the surface plug from 1484 feet to 1284 feet. Cement was displaced with 20.5 barrels of fresh water. RBD tripped 31 joints of 4.5 inch diameter casing and fluid level was at 176 feet. BWS tripped remaining casing from wellbore and Formation Cementing Inc. pumped a 45 sack 176 foot Class A Surface plug from 176 feet to surface circulating cement to surface. Instructed operator to cut casing off 30 inches below grade and install metal plate identifying well and reclaim according to ORC 1509.072B.

1419800271 2017-06-26 Plug / Plug Back No Violations On the day of my inspection I witnessed RB Drilling (RBD) was onsite rigged up and completing pit excavation and lining. RBD swabbed the well down to the top perforation at 5544 feet with 5 pulls. Shut-down to wait on mix water and cementers. Formation Cementing Inc. arrived and bullheaded a 30 sack (6.4 barrels) Class A Clinton cement plug from 5594 feet to 5257 feet and squeezing one barrel away. Cement was displaced with 83.5 barrels of brine and the well was shut-in. Utilizing an ODNR Rental Ventis meter revealed Hydrogen Sulfide (H2S) levels at 0. Also utilizing ODNR Tango H2S detector revealed H2S levels at 0.
1498738092 2017-06-29 Plug / Plug Back No Violations On the day of my inspection I witnessed the top cement plug was at 30 inches from surface. Pit closure was underway with casing to be cut off 30 inches below grade. Well identification was present and will be placed on casing with site reclamation to begin later today.
