2041078078 2017-02-21 Production Wells No Violations On the day inspection at 08:32, I received a call from Terry Chapman who stated that a gas and oil leak was near his property. Upon arrival at 08:40 and following up on complaint ID 5539, I met with Mr. Chapman who escorted me to the leak. I witnessed a 2 inch diameter riser from a 4 inch diameter gathering line was leaking gas and oil at Global Positioning System Coordinates 40.048906 -81.456476. The oil contamination was less than a barrel and covered an area 7 feet wide by 60 feet long reaching the edge Beeham Run Creek. I placed a call to Enervest Superintendent Tim McConaha and stated my findings. Mr. McConaha stated that a crew would arrive within the hour. Enervest employees arrived at 09:15 and began to remediate the contamination and repair the line. Enervest employees removed all oil contamination and applied bio-sorb to the affected area with completion of work at 12:00 and resolving the complaint. At 12:15 I met with Ohio Environmental Protection Agency (OEPA) Emergency Response Chris Holmes and re-visited the site and reviewed clean-up measures taken. I then inspected the said well and witnessed the production equipment included a 100 barrel tank, a horizontal separator, a drip, a 2 inch sales meter with a .75 inch orifice and chart recorder all within an earthen dike. The well was equipped with 4.5 inch diameter casing, 1.5 inch diameter tubing, a plunger lift system and domestic gas service with valves open on the tubing and valves open on the casing supplying the motor valve and domestic gas. Production records were current and identification was present.
