0240670938 2015-05-29 Plug / Plug Back No Violations On the day of my inspection I witnessed that Berry Well Service ran their tools down the 3.5 inch casing to 5250 feet and tripped out of the hole. Appalachian Well Service was on site and was unable to perforate the Big Lime as water level was at 4750 feet and BWS water truck was not in operation. BWS will trip 1.5 inch tubing to 5250 feet and commence with plugging operations on Monday.
0663365193 2015-06-01 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Austin Gutridge, I witnessed Berry Well Service had tripped 1.5 inch tubing to 5220 feet. Formation Cementing was on site and pumped a 400 foot 17 sack Class A bottom hole plug from 5220 feet to 4820 feet. BWS pulled 50 joints of 1.5 inch tubing and Appalachian Well Service perforated the Big Lime with 10 Shots from 3770 feet to 3760 feet. BWS ran 4 joints of 1.5 inch tubing back in to 3766 feet and Formation pumped a 200 foot 57 sack Class A Big Lime plug from 3766 feet to 3566 feet. BWS pulled remaining 1.5 inch tubing from wellbore and the Berea kicked off on the backside of the casing. BWS loaded the 3.5 inch casing and Appalachian Well Service shot 3.5 inch casing off at 2400 feet. BWS shut-in the 3.5 inch casing and will resume plugging operations on Tuesday 6/2/15 with Formation circulating the backside prior to pulling the casing and setting the Berea plug.

1433250725 2015-06-02 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Austin Gutridge, I witnessed Formation Cementing circulated on 3.5 inch casing with 10ppg brine to kill the well. Berry Well Service pulled 33 joints of 3.5 inch casing and Formation pumped a 203 foot 55 sack Class A Berea plug from 1376 feet to 1173 feet. BWS pulled 24 joints of 3.5 inch casing and Formation pumped a 200 foot 58 sack Class A Bottom of the surface plug from 624 feet to 424 feet. BWS pulled 14 joints of 3.5 inch casing and Formation pumped a 200 foot 60 sack surface plug from 200 feet to surface. BWS pulled remaining casing and plug dropped. Formation topped off plug to surface with 38 sacks and cement stayed at surface. Instructed operator to cut casing off 30 inches below grade and install metal plate identifying well and reclaim according to ORC 1509.072B.

2069927897 2015-05-28 Production Wells No Violations On the day of my inspection I met Berry Well Service and located well for upcoming plugging. The production equipment included a 100bbl tank, a horizontal separator, domestic gas service with valves in the closed position, a 2 inch sales meter with a .25 inch orifice and a chart recorder all within an earthen dike. The well was equipped with 8.625 inch casing and 3.5 inch casing reduced to 2 inches with valves in the closed position.
