There was no change in the status of the well since the last inspection on 12/12/17 and there has been no communication from Green Gas regarding plugging of the well.
0432637058 2017-07-06 Production Wells Violations Noted On the day of my inspection I witnessed the well was found in heavy vegetation and equipped with 4.5 inch diameter casing reduced to 2 inches with the valve in the closed position. Abandoned power poles were present with no flowline found and no identification. Records indicate the well was plugged back and no production has been reported since 1993. The production equipment included 2-210 barrel tanks and a 37 barrel polymer tank within an earthen dike. Contaminated soil was found near the burner flanges on the 210 barrel tanks and near the polymer tank. The well Owner/Operator shall remove defective tanks, remove contaminated soils and dispose of in an approved landfill. The well Owner/Operator shall also submit a written plugging schedule to Doug Bennett at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

During my inspection the well was equipped with 4.5 inch diameter casing and a 4.5 inch to a 2 inch diameter swage nipple with a 2 inch valve in the closed position. Green Gas has submitted reports of zero production for the years 1993 and 2017.

I met with land owner Mr. and Mrs. St. Clair who own 34059213760000 St. Clair 1. Mr. St. Clair stated he owns the east 210 barrel tank within the dike. The west disconnected 210 barrel tank and the polymer tank are the tanks which are leaking.
