0354114971 2017-11-02 Plug / Plug Back No Violations On the day of my inspection with rain, I witnessed Steve Hagen Well Service (SHWS) had tripped 1.5 inch diameter tubing in to depth of 4620 feet. Petroset Cementing Services arrived, had to be pulled in to the wellsite and rigged up. Petroset pumped a 30 sack 400 foot Class A bottom hole plug from 4620 feet to 4220. Cement was displaced with 11.1 barrels of fresh water. SWHS tripped 1.5 inch diameter tubing from wellbore and could not obtain telegraph in the Big Lime. Appalachian Well Surveys (AWS) was called out and tripped inside the 4.5 inch diameter casing finding fluid level at 1100 feet. AWS shot the 4.5 inch diameter casing at 3365 feet.

0644449902 2017-11-03 Plug / Plug Back No Violations On the day of my inspection with rain, I witnessed Steve Hagen Well Service (SHWS) and Petroset Cementing Services were onsite. SHWS pulled 50,000 on the pipe and could not get it freed out of the slips. Petroset pumped on the 4.5 inch diameter casing which was shot off at 3365 feet gaining circulation with 45 barrels of fresh water. Petroset pumped a 200 foot Class A Big Lime cement plug from 3365 feet to 3165 feet. Cement was displaced with 50.3 barrels of fresh water. SHWS pulled 4.5 inch diameter casing to 1500 feet and Petroset pumped a 55 sack 200 foot Class A Bottom of the surface cement plug from 1500 feet to 1300 feet and displace cement with 20.6 barrels of fresh water. SHWS pulled 4.5 inch diameter casing to 200 feet and Petroset pumped a 75 sack 200 foot Class A Surface plug from 200 feet to surface. Instructed operator to cut casing off 30 inches below grade and install metal plate identifying well and reclaim according to ORC 1509.072B.

1497615430 2017-06-16 Production Wells No Violations On the day of my inspection the well was equipped with 8.625 inch diameter casing and 4.5 inch diameter casing reduced to 2 inches with valves in the closed position. The production equipment included a 100 barrel tank which did not meet the spacing requirements set in ORC 1501:9-9-05 (A) (2). Also present a 2 inch salesmeter with a .125 inch orifice and a chart recorder with no chart present all within an earthen dike. A plug permit was issued for the well on May 3, 2017. Identification was present and production records were current.
1531828664 2018-07-17 Final Restoration No Violations The purpose of my inspection was status check on a final restoration. During my inspection all equipment and debris have been removed and vegetation had been established. A landowner waiver had been presented to me by Kenoil. The landowner waiver stated that Kenoil is not responsible for reclaiming the lease road and well pad. The waiver had the landowners signature that was necessary to validate the form. The lease road and well location were left at the original grade as the landowner waiver had stated and the areas appeared to be smooth graded with thick vegetation in place.

SHWS tripped inside the 4.5 inch diameter casing setting down at 4650 feet. SHWS tripped out and began running 1.5 inch diameter tubing to set the bottom plug tomorrow.
