The production equipment included a 210 barrel tank within an earthen dike. The well was equipped with 8.625 inch diameter casing, 4.5 inch diameter casing, 2.375 inch diameter tubing, rods and an Alten pumping unit powered by natural gas with all valves in the open position. The plug remains missing from the 2 inch load line of the 210 barrel tank with a small drip resulting in a 3 foot diameter of contamination. Call was placed to Mark Van Tyne of Pin Oak requesting an update. Mr. Van Tyne stated that a rig would be onsite late spring.

Communication received today from Pin Oak states that are trying to transfer the well to the land owner and there is a hole in the tubing. If the land owner does not take the well repairs will be made by May of 2018 and the well will be back online.

The lease road to the well had recently been re-graded. Following my inspection, I called Allan Berry with Berry Enterprises to discuss the plans for the well. Mr. Berry stated that he will be bringing a service rig to service the well and bring it back into commercial production. The service rig is expected to arrive within the next week.

1506604463 2017-09-28 Production Wells Violations Noted On the day of my inspection following up on Complaint ID 5712, I witnessed the production equipment included a 210 barrel tank within an earthen dike. The well was equipped with 8.625 inch diameter casing, 4.5 inch diameter casing, 2.375 inch diameter tubing, rods and an Alten pumping unit powered by natural gas with all valves in the open position. Identification had been updated since the last inspection and production records were current with zero production reported since 2013. The plug was missing from the 2 inch load line of the 210 barrel tank with a small drip resulting in a 3 foot diameter of contamination. The well owner/operator shall submit a written plugging schedule to Doug Bennett at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. The well owner/operator shall install a 2 inch plug in the load line, remove contamination and replace wit clean fill.
1619133867 2018-10-25 Production Wells No Violations The purpose of my inspection was a follow up on Compliance Notice 1506604463 and Complaint 5712. The compliance notice stated that there was a continuous drip coming off of the load line from the 210-barrel tank and that the well was idle. During my inspection today, the well was equipped with 8.625-inch diameter casing with a casing head, 4.5-inch diameter production casing with a tubing head, 2.375-inch diameter tubing, rods, and a pumping unit. The production valves were in the closed position. A domestic gas line was attached to the 4.5-inch diameter production casing with an open valve, a Little Joe regulator, and a drip. I spoke with the land owner Frank Whitehair and he stated that the well is being used for domestic gas. The production equipment consisted of one 210-barrel tank. A bull plug had been installed in the load line to prevent leaking. I spoke with complainant Ted Fehrman and he stated that his compliant is satisfied. The compliance notice is also satisfied now that the bull plug has been installed and the well is production domestic gas.
