0232373594 2014-02-10 Production Wells No Violations On the day of my inspection following up NOV-1357818813, the ground was snow covered and I witnessed that compressor unit along the lease road was not running but the power was switched to auto. The lease road was gated and locked and the well was equipped with 8.625 inch casing, 4.5 inch casing, 2.375 inch tubing, a plunger lift system and a logic controller. The valves were open on the flow line and opposite the flow line the valve was open to the atmosphere. The tank battery consisted of a 2-100bbl tanks which did not meet the minimum spacing requirements of 36 inches vertical separator. Identification was present but not legible.

The tank site consists of [2]100-barrel steel tanks placed within 8 inches of each other and 1 vertical separator with no pressure on the unit. The tanks are enclosed within an earthen impoundment and there was no identification posted. The briars have grown up around the production equipment and the well. The gas was till venting into the atmosphere. There was a sour gas smell but could not get readings with the Ventis pro 4 gas meter.

The company has 5 days to install the bull plugs needed and 5 days to prevent the well from emitting H2S from the well.

1357682473 2013-01-08 Administrative Inspection No Violations As per Mike Hartshorns inspection revealing that well is capable of producing resolving previous compliance notice issued for idle well owner has complied with issue and has placed dike around tanks but has failed to correct spacing requirement and has not properly identified well and has not complied with request to report production as required by law, local inspector Mike Hartshorn will reissue notice for tank spacing requirement and to post proper identification.

Note: Received notice from landowners attorney stating that lease has been cancelled and current owner has no right to attempt to produce well.

1357818813 2013-01-10 Production Wells Violations Noted On the ninth day of January 2013, I made an attempt to contact Big Sky Petroleum LLC and left a message with Bob Barr to let him know that enforcement may be required due to lease cancellation on pursuant to affidavit of forfeiture see O.R. 491 Pg. 1339 see O.R. 493 Pg 1232. I left a message on Bob Barrs voice mail to return my call. On the tenth of January 2013 I received a phone call from Bob Barr, he informed me that he complied with the state and the well was back in production. After speaking with Mr. Barr I drove to the well site to do an inspection, the inspection revealed at the well head the two inch gate valve was open as well as the valve on the production line. There is no identification posted, there has been no production records recorded since the year of 2009. There is a compressor at the beginning of the entry way leading back to the well site. The violations at the well site are, / 1501: 9-9-05(A)10 failure to legible identify well/ 9-9-05(A)2 violation of tank spacing/ 1509.11. Statement of production. The requirements to meet the divisions standards will be, 1. Properly identify the well 2. Meet the minimum requirements of three foot spacing between the tanks. 3. Meet the divisions standards for the statement of production.

--105, and Compliance Notice -1357818813. During our inspection today, we traveled the heavily eroded lease road on foot back to the well. The well was equipped with 8.625-inch diameter surface casing with a casing head, 4.5-inch diameter production casing with a tubing head, 2.375-inch diameter tubing, and a plunger lift system. All valves were in the closed position. The Demco master valve was in the closed position on the tubing and no lines were attached to the production casing. A sour gas smell can be identified by the wellhead. While utilizing the Ventis pro 4 gas detector no Hydrogen Sulfide Gas recordings were detected. The production equipment consisted of two 100-barrel tanks and one vertical separator. The two 100-barrel tanks were within 3-feet apart and did not meet spacing requirements stated in the Ohio Administrative Code 1501:9-9-05(A)(2). Identification was not posted within a conspicuous place near the wellhead or production equipment.
