Contamination needs to be removed, remediated and replaced with clean fill. The contaminated soils need to be disposed of in an approved landfill and manifest need to be provided to ODNR upon disposal along with the well identification updated. |

Production equipment included a 100 barrel tank which had a drip on the 2 inch valve resulting in a 2 feet by 12 feet area of stressed vegetation. No identification was present and production records were current. Owner/Operator shall properly identify well in accordance with OAC 1501:9-9-05 (A) (10). Owner/operator shall construct a SPCC dike and install plugs or locks where instructed, remove contamination, replace with clean fill and dispose of properly in a solids landfill and provide ODNR with the manifest for the disposal. |
