0298975161 2017-12-14 Production Wells No Violations On the day of my inspection I met with the land owner Mrs. McManaway who stated the well was in her field and that she and her husband were the owners of the well. The well was equipped with 7 inch diameter casing and 4.5 inch diameter casing reduced to 2 inches with the valves in the closed position. No flowline or identification was present. I met again with Mrs. McManaway who stated she would have her husband call me to discuss the well. I placed call and advised the owner of record to call me to discuss the well. On 12/20/17 Linda Dever returned my call and stated that she was unsure of where the well was located and that she has no documentation on the well. Ms. Dever will be contacting the landowners to sort out ownership and either get the well back online or apply to have the well plugged.
1311304145 2019-04-10 Production Wells No Violations At the well site today with ODNR inspector Rick Brown and Knox Energy representative Bill Minton to discuss plugging operations. During my inspection a single pole service rig was rigged up on location. Mr. Minton present to me a plan to plug the well and stated that he might try and commence operations on 4/12/19 depending on the weather.
1449235416 2019-04-15 Plug / Plug Back No Violations Appalachian Well Surveys shot the 7-inch diameter casing at 942-feet with a chemical casing cuter. The shot fired but the casing was not free. The wellhead was built back up off of the 7-inch casing and 1.5-inch diameter tubing was tripped within the 7-inch diameter casing. The wellhead was packed off and Cemenco plumbed up to the 1.5-inch diameter tubing and pumped 50 sacks of class A cement from 930-feet to 680-feet. The tubing was tripped up above the cement and Cemenco plumbed back up to the 1.5-inch casing and pressure up with fresh water to squeeze cement through the hole from the casing cutter at 942-feet. The 1.5-inch tubing was tripped from hole. 2 joints of 7-inch diameter surface casing were backed off from surface. Cemenco pumped a 100-foot surface plug within the 10-inch diameter conductor. The cement stayed at surface and Berry Well Service was instructed to cut the conductor down 30-inches below surface grade.

1944319853 2019-04-16 Plug / Plug Back No Violations Berry Well Service was on location with a single pole service rig to complete plugging operations. The well was drilled to a total depth of 5100 feet to produce the Clinton Sandstone. The well was equipped with 23 feet of 10-inch diameter conductor, 7-inch diameter surface casing set at 1504-feet cemented with 15 sacks and then the remainder of backside was mudded to surface, and 4.5-inch diameter production casing that was cemented with 300 sacks by Halliburton. The well was completed through the 4.5-inch diameter production casing with 18 perforations from 4988-feet to 4952-feet. (4/16/19) During my inspection Cemenco cementing pumped a 30 sack plug with class A cement from 4950-feet to 4550-feet. Appalachian Well Surveys arrived on location to verify a free point and remove the 4.5-inch production casing from the 6.25-inch diameter borehole. The estimated top of the 300 sacks of cement on the back side of the 4.5-inch casing came to 1660-feet. Appalachian Well Surveys ran a bond log and the free point was found, and a casing cutter parted the casing at 2440-feet. Two 32-foot joints of 4.5-inch casing were tripped from hole and Cemenco pumped a 40-sack class A cement isolation plug from 2380-feet to 2180-feet within the 6.25-inch diameter borehole. The 4.5-inch casing was pulled up to 1600- feet and Cemenco pumped a 40-sack class A cement plug from 1600-feet to 1400-feet a crossed the bottom of the surface casing 100 feet within the 6.25-inch borehole and 100 feet within the 7-inch diameter surface casing. The 4.5-inch casing was tripped from hole.
1947832790 2018-03-30 Production Wells No Violations On the day of my inspection I witnessed there was no change in the well since the last inspection on 12/14/17. The well was equipped with 7 inch diameter casing and 4.5 inch diameter casing reduced to 2 inches with the valves in the closed position. A plug permit was issued on 3/2/18 and call placed to Linda Dever revealed that she plans to have the well plugged this summer.
